Patients with uterine fibroids who have fertility requirements should first determine whether the current fibroids are affecting fertility, because there are many reasons affecting fertility. When there is a clear history of infertility, a systematic examination should be conducted at a reproductive specialist to try to clarify the causes of infertility and to provide targeted treatment. Not all fibroids can cause infertility, for example, subplasma fibroids, unless they are very large and press on the fallopian tubes, are generally not likely to cause infertility; interstitial fibroids and submucosal fibroids are more likely to cause infertility or sterility if they grow to a certain size. Women with fibroids who have not conceived after more than six months of normal pregnancy preparation, or who experience embryonic arrest or spontaneous miscarriage early in pregnancy, should consider whether the fibroids are the cause of their infertility, among other reasons, and may need to be treated for the fibroids. Myomectomy, or myomectomy, is the traditional method for dealing with fibroids with fertility requirements, and can be open or trans-lumpectomy, both laparoscopic and hysteroscopic. For subplasmalemmal fibroids with tissues or submucosa with tissues, excision is a very safe and effective treatment of choice, but for other types of fibroids, excision inevitably brings about an incision in the muscle wall or lining, posing a risk for subsequent pregnancy and fertility, so that re-pregnancy after treatment often takes one to two years. Ultrasound ablation technology (Helio knife) is different. When it treats fibroids, there is no incision not only on the abdominal wall but also on the muscle wall or endometrium, therefore, the uterus after fibroids treated with ultrasound ablation can be prepared for pregnancy earlier after the treatment, usually there is no special after the review in 3 months after the treatment, and at the same time, there are more and more cases at home and abroad showing that patients with fibroids treated with ultrasound ablation only can mostly have spontaneous and normal delivery after pregnancy.
