The most prominent symptom of advanced uremia due to diabetic nephropathy is high edema. Due to the huge protein leakage from diabetic nephropathy patients, it can lead to severe hypoalbuminemia, which can cause severe edema. Besides subcutaneous edema of the whole body, it can also cause edema in all parts of the body cavity. In case of severe pleural cavity effusion, it can lead to chest tightness, shortness of breath, and difficulty in breathing. In case of severe peritoneal effusion, it can lead to loss of appetite, abdominal distention, nausea, and vomiting. In case of pericardial effusion, there may be panic, palpitations, chest pain, and dyspnea. Other than that, other symptoms are basically the same as ordinary uremia, mainly manifesting as metabolic acidosis, electrolyte disorders, renal hypertension, renal anemia, renal bone disease, etc.
