Liver lobe atrophy belongs to the traditional medical term “swelling”, which has been considered by doctors for many generations to be caused by the stagnation of qi, stasis of blood, and accumulation of water. As stated in the chapter of Qing Dynasty – He Mengyao’s “Medical History – Swelling”: “Qi, water, and blood are always related to each other, and there are those who suffer from Qi stagnation first and then blood defeat, those who suffer from blood stagnation first and then Qi stagnation, those who suffer from edema first and blood defeat, and those who suffer from blood stagnation first and then water storage. Modern medicine believes that hepatic lobe atrophy is the decompensated stage of cirrhosis, which is an inevitable pathological result of cirrhosis first enlarging and then shrinking. Although hepatic lobe atrophy also appears in the late stage of hepatitis, but the etiology and pathogenesis of hepatomegaly are completely different. First, the liver and kidney yin deficiency, water does not help fire, false fire on the inflammation, the liver body by scintillation and atrophy. Therefore, the symptoms are gingival, epistaxis with blood, red lips, heart trouble and insomnia, red tongue with little or no moss, dry and burning stool, and thin pulse. Secondly, liver yin deficiency and damp-heat fumigation, the fluids are scorched, the liver body is not nourished, so it gradually atrophies. Therefore, the face is dull or the sclera skin is yellowish, the mouth is bitter or dry, the heart is irritable, the urine is yellow and the stool is dry or soft and loose, the tongue is thin and small, the tongue coating is yellow or yellowish. Third, Yin deficiency fire or Yin deficiency damp heat first causes liver atrophy, and in the course of treatment overuse of bitter-cold products damages stomach yang, resulting in lack of movement of the middle palace and internal stagnation of water-dampness, so ultrasound or CT examination shows liver lobe atrophy without obvious clinical symptoms of Yin deficiency fire or Yin deficiency damp heat. The symptoms include dry mouth and lips, insomnia and sleeplessness, abdominal distension, ascites, fatty tongue, white greasy coating, loose or soft stools, and slow pulse.
