Why are elderly patients prone to recurrence after hernia surgery?

After hernia repair, the hernia can still have a certain recurrence rate depending on many factors such as the patient’s age, physical condition, the size of the hernia mass and the chosen surgical approach, operation and whether it is accompanied by increased intra-abdominal pressure and incisional infection. Especially in the elderly, recurrence is more likely after surgery. It is generally considered to be related to the following factors. 1, weak abdominal wall The elderly generally have fewer activities and often lack the necessary abdominal muscle exercise, which can lead to degenerative lesions of the abdominal wall muscles, making the abdominal muscles gradually weak. When the occurrence of long-term cough, urinary difficulties and other reasons caused by increased intra-abdominal pressure, viscera can protrude through the abdominal wall muscles, forming an extra-abdominal hernia or cause recurrence of hernia. 2. Long-term constipation In elderly people, long-term constipation can be caused by reduced tone of the colon smooth muscle, abdominal muscle, diaphragm and levator muscle, weakened rectal reflex, as well as poor dietary and bowel habits and weakness, which can increase intra-abdominal pressure and make it easy to recur after hernia repair. The elderly have poorer immunity, atrophy of the respiratory mucosa, lower secretion function, and slower defense reaction of the larynx, as well as atrophy of the bronchial mucosa. Recurrence. Difficulty in urination Prostate enlargement, bladder tumor, stone or prostate cancer can often cause difficulty in urination or even urinary retention. Different degrees of difficulty in urination are often factors that increase intra-abdominal pressure and cause easy recurrence of postoperative hernia. Therefore, before performing hernia repair, elderly people should actively treat various diseases that increase abdominal pressure, such as preventing respiratory infections, controlling prostatic hyperplasia, and keeping bowel movements unobstructed, etc. At the same time, the strength of abdominal muscles can be strengthened by means of exercise to reduce the chance of recurrence after surgery. The main exercise methods include sit-ups, flat leg lifts and abdominal massage. After surgery, special attention should be paid to rest, especially within 1 week after surgery, except for the necessary daily activities, and bed rest as much as possible. It is not advisable to carry weight or do exercises that increase intra-abdominal pressure too early to avoid affecting the normal healing process and causing recurrence of hernia.
