Patients with high myopia who want to get rid of their glasses

  Every year, a large number of myopic patients come to me with the main goal of getting out of glasses and still being able to see clearly. For most patients with mild myopia, I recommend continuing to wear glasses because they are not too high, they are comfortable to wear, and they have a certain aesthetic effect. For modern people who have been looking mainly at the near for a long time, mild myopia can also make people not very fatigued when they use their eyes at close range for a long time. For moderate myopia, my advice is to consider wearing glasses or laser surgery, wearing glasses is simple and cheap; doing laser surgery, the effect and safety are good, is a little expensive.  But for patients with high myopia and moderate myopia who cannot have laser surgery, there are very few options available. These patients need the most treatment. Patients with high myopia are not good with glasses because of high prescription, low prescription, although the glasses are lighter, but can not see clearly, the prescription is enough, although see clearly, but the glasses are heavy, the effect of seeing things is not good, the aberration is more obvious. These patients are not yet suitable for laser surgery, because compared with moderate myopia, the safety of laser is significantly reduced.  So is there a solution to the plight of highly myopic patients?  I would like to offer you 3 options: 1. If you are financially able, I would recommend posterior chamber IOL implantation, simply called “ICL” implantation surgery. This technology has a history of more than 10 years. The US FDA has conducted a review of the results and complications of this type of surgery over the past 10 years and found that for high myopia, this surgery is very effective and safe, and is a more suitable method for high myopia.  2. If you are older and already have cataracts, especially posterior subcapsular cataracts that are prone to high myopia, you can have cataract extraction + IOL implantation; that is, by treating cataracts and implanting IOLs of special degrees at the same time, high myopia can be treated together, and eventually a more satisfactory vision and visual effect without eyes can be formed. Since cataract-related surgery is now very mature with few complications and excellent postoperative result control, I suggest that if you already have a cataract and high myopia, you should not wait for the cataract to mature and have early surgery to live a good result. For the problems related to cataract surgery, I will not go into details because there are very many related information. In short, cataract related surgery, if there is a standardized process and management, the safety and effect are very good now, so there is no need to worry.  3, rigid corneal contact lens: this technology for many years, similar to our common contact lenses, most of them are used for the treatment of mild myopia, and now some experiments have started, mainly used in patients with high astigmatism, for patients with high astigmatism combined with myopia, the effect is still good. However, the main problem with this technique is that it must be worn every day, and if you stop for a while, the astigmatism and myopia will go back.  Here I will talk about the first method in detail: posterior chamber IOL implantation for crystalline eyes. The principle of this surgery is simply to make the glasses lens small enough and fine enough, and then put it inside our eye through a small incision of about 2mm at the edge of the black eye, behind the pupil, so that it can take the place of the glasses. Because the IOL material used is special and the prescription is specially selected or customized according to each individual’s prescription, the surgery requires a very experienced surgeon to do it, so this seemingly simple surgery is not widely carried out in China at present, mainly in Shanghai, Beijing and Guangzhou.  First of all, let’s talk about the safety issues that concern people the most.  1. Will the IOLs placed in the eye fall out or be off to the side? The IOL is placed in the posterior chamber of the eye, the front is pressed by the pupil, the back is held by the lens, and the ciliary sulcus is fixed on the side. Actually, no, there is a large space in the eye for the intraocular fluid to circulate in order to maintain the normal activities in the eye, and the IOL is very thin and placed in a gap, so it will not be too crowded; however, in order to let the intraocular fluid circulate more safely, a laser punch is usually needed before the surgery to create a channel for the flow of water, just in case.  3. Which is more damaging, this surgery or laser? The principle of laser is to cut off a piece of the black eye (cornea) to treat myopia, if more is removed, the cornea becomes too thin, just like a balloon where some parts are very thin, if blowing, the very thin parts are very easy to bulge out and break, that is the main reason why highly myopic patients are not suitable for laser; while implanting an IOL, at least not to remove the eye itself, the incision If there is a problem with this IOL, it can be removed, and if other eye diseases occur in the future, such as cataracts, it can be removed without affecting the later treatment; of course, this surgery is not completely free of danger, and there is a risk of inflammation even though the incision is small. However, doctors currently have many methods to prevent infection, so that the possibility of inflammation is as small as possible.  4.How many years does an IOL last and do I need to change it in the future? Generally speaking, as long as there are no special problems, IOLs are guaranteed for life and do not need to be replaced.  Then we will talk about the advantages of this technology.  1. It does not harm the tissues of the eye. Unlike laser surgery and other corrective surgeries, implanting an IOL hardly hurts the tissues of the eye, which ensures the safety of this technology. 2. It retains its own adjustability. Compared to cataract surgery, this type of surgery retains the ability to adjust because it retains its own lens, so that it can achieve clearer results after surgery, both near and far, than after general cataract surgery. The vast majority of patients no longer need glasses to achieve good results; 3. Better visual quality. Most of us are still at the basic stage of vision awareness like the vision chart. In fact, the same 1.0 vision, with and without astigmatism, large and small aberrations, the clarity and comfort felt by patients themselves are different. For patients with high myopia, glasses produce more significant aberrations that affect the quality of vision, while the IOL implanted in this procedure has much less aberration than glasses, so the quality of vision is much better than glasses, and many patients can obtain better vision than with glasses.  In conclusion, for patients with high myopia who wish to go without glasses, they can consider a posterior chamber IOL implantation for crystalline eyes, and the results are usually good as long as the doctor treats them carefully and responsibly.
