What are the indications for adjuvant chemotherapy after gastric cancer surgery?

  (1) Patients with T2N0 who have high-risk factors (low tumor differentiation or high histologic grade, lymphovascular infiltration, neural infiltration, and age less than 50 years) should receive adjuvant radiotherapy and chemotherapy after surgery, and T2N0 patients without high-risk factors can be followed up and observed; (2) Patients with T3, T4 or any concomitant lymph node metastasis (i.e., N1-3) who have achieved RO resection should receive radiotherapy and chemotherapy . T2 management for cardia cancer is the same as that for distal gastric cancer after Ro resection; (3) Patients with both R1 resection and R2 resection without metastasis should receive postoperative radiotherapy (45 – 50Gy) plus 5-FU-based chemotherapy.
