Clavicle hook plate applied to distal clavicle fracture

  Currently, clavicle hook plates are most popular for distal clavicle fractures.  The main advantages of this plate include: 1. solid and firm fixation, which is sufficient to maintain the stability of the fracture end. 2.  2. It can promote the healing of the fracture, ensure the early movement of the shoulder joint, and prevent the joint from being restricted.  3. Intraoperative repair of the rostral collateral ligament, firm fixation is beneficial to the healing of the ligament and does not loosen or retract.  Disadvantages: 1.The price is too expensive, and the patients with distal clavicle fracture are mostly manual laborers with limited economic capacity.  2. The plate shape is large and requires too much exposure during surgery, which is traumatic to the surrounding tissues.  3.The postoperative incision scar is long and affects the aesthetics.  4.The acromioclavicular joint is a micro-movement joint, and the fixation of the clavicle hook plate will affect the function of the shoulder joint.  5.It can stimulate the subacromial bursa, and the shoulder joint supination will be limited.  I have some different opinions. For shoulder lock joint dislocation, you can choose simple internal fixation treatment or simple ligament reconstruction surgery. It is my personal custom to detect the integrity of the rostral ligament while internal fixation of the hook plate. If intraoperative rupture of both the tapered and trapezius ligaments is found, it is recommended to reconstruct the ligaments at the same time and pay attention to repairing the acromioclavicular capsule.  Exceptionally, the hook plate is designed with the concept of micro-movement fixation, which basically does not affect the movement of the acromioclavicular joint. The appearance of impingement symptoms is mostly due to the anterior placement of the hook.
