What is the significance of the test for insulin-like growth?

  In the guidelines for the treatment of children with short stature in China, it is recommended that these two hormone tests should be routinely performed in hospitals that have the conditions, but many parents do not understand the significance of these two tests. The following is a brief explanation of these two tests.  We all know that the growth of children’s height is closely related to the secretion of growth hormone, a hormone in the human organism, and if this hormone is lacking, they will suffer from pituitary dwarfism and will be very short in stature as adults. That is why we perform growth hormone measurement (growth hormone stimulation test) for short children.  On the one hand, growth hormone acts directly on bones to promote bone growth, which is the growth of human body; on the other hand, it acts on liver growth hormone receptors to promote the synthesis of IGF-1 in the liver, which promotes bone growth through IGF-1. At the same time IGF-1 feedback inhibits the release of pituitary growth hormone. The concentration of serum IGF-Ⅰ and serum growth hormone level are roughly parallel within 24 hours.  Therefore, IGF-1 decreases when growth hormone is deficient. IGF-1 secretion also decreases when liver function is abnormal, so children with severe liver disease will have their height affected. Then there is the child with growth hormone insensitivity syndrome, who is not deficient in growth hormone, but cannot produce enough IGF-1 and will be short in stature.  Children treated with growth hormone also need to be tested for IGF-1 at the same time, and the ideal effect of medication is to control IGF-1 to the high normal limit.  To sum up, it is necessary to test IGF-1 in children with short stature.  At present, there is no national unified IGF-1 reference value in China, mostly using the reference value provided by the experimental reagent manufacturers.  Meanwhile, at present, the national pediatric endocrinology and genetic metabolic disease group, still recommends using the results of the IGF-1 and IGFBP-3 normal standard study of Shanghai Xinhua Hospital as the main reference basis.
