Diabetes is a common disease that endangers human health. With the improvement of people’s material living standard, the incidence of this disease is increasing year by year. At present, diabetes has become one of the three major chronic and difficult diseases that cause the most headaches for urban and rural residents. In recent years, the clinical application of continuous subcutaneous insulin infusion (CSII) and insulin pump has become one of the important advances in the field of diabetes treatment in order to better control the blood sugar of diabetic patients and reduce the pain of insulin injection. Confusion of diabetic patients For a long time, diabetes, as a chronic and difficult disease, has troubled many people: taking Chinese medicine, the effect of lowering sugar is slow; taking Western medicine, although the effect of lowering sugar is fast, but long-term use of liver and kidney has certain side effects; using a syringe to inject insulin, three times a day, not to mention the pain, long-term injection is easy to cause infection, and the injection dose is not easy to control. Because blood sugar is not effectively controlled, diabetic patients with long-term illnesses are prone to various complications and even threaten their lives. Although the cause of diabetes is very complex, it is ultimately caused by absolute lack of insulin, relative lack of insulin or insufficient insulin effect, so how to effectively replenish insulin and control blood sugar has become the key to treatment. As a result, many diabetic patients began to hope for a new method of blood sugar control that is simple, effective and can reduce the appearance of various complications. So, is there such a new treatment? With this question, the reporter interviewed Sun Liangge, chief physician of the Department of Endocrinology at the First Affiliated Hospital of Zhengzhou University. The insulin pump has been applied Sun Liangge said, “In the treatment of diabetes, there is indeed a new instrument that can effectively control blood sugar and reduce the appearance of complications – insulin pump.” Insulin pump is a peristaltic pump controlled by a microcomputer, which can continuously inject insulin in micro amounts. It can simulate the normal human pancreatic insulin secretion curve, through a special microtube insulin continuously and variably into the subcutaneous, within 24 hours of continuous control of blood sugar in the normal range, thereby reducing the occurrence of complications in diabetic patients. Sun Liangge said: “Now widely used insulin injection technology can also effectively supplement the insulin required by diabetics, but the injection method of several fixed times a day does not fully meet the physiological needs of the human body, the insulin pump can effectively solve this problem. Because of its microcomputer power device, it can imitate the human physiological rhythm, with the human physiological needs to release the required insulin on its own, and can automatically add insulin according to the post-prandial peak, so it is a very good extracorporeal artificial pancreas.” It is understood that the insulin pump is the best therapy for the treatment of diabetes at present. Indications for insulin pumps The reporter learned in an interview that all the indications that require insulin are the indications for insulin pumps. In other words, type I diabetic patients and part of type II diabetes can use insulin pumps. Type II diabetic patients can use the product in the following cases: by diet control and exercise therapy, and various oral hypoglycemic therapy is ineffective; acute complications, such as ketoacidosis, hyperosmolar coma and other conditions; combined with tuberculosis; combined with serious liver, kidney, heart, brain and eye complications; pregnancy. Sun Liangge said: “The first case of insulin pump in China was applied to the clinic in 1998. Although it has not been used for too long in China, it is technically mature because it has been widely used in Europe and the United States.” Sun Liangge said, “After the insulin pump was applied to the clinical treatment of diabetic patients in our province, it has produced positive efficacy in the control of fundus lesions in patients and the disappearance of trace protein in early nephropathy.” The U.S. DCCT study showed that the emergence of various chronic complications can be delayed by about 15 years with the use of insulin pumps for intensive diabetes treatment. The price of insulin pump is not expensive According to the reporter, as a new medical product that has not entered our market for a long time, the price and application cost of insulin pump is still relatively high, which also affects the use and promotion of insulin pump. According to Sun Liangge, the current market price of insulin pumps in our province ranges from 25,000 to 55,000 yuan. At the same time, the long-term use of insulin pump injection catheter is prone to blockage and pollution, and needs to be replaced once every 7 to 10 days. According to the patient’s condition, insulin must be added to the pump regularly, so in addition to the initial cost of purchasing the insulin pump, the patient needs to spend 350 to 500 yuan per month for daily expenses. Precautions for application At present, the insulin pumps used in China are basically open-loop pumps, which require patients to monitor their own blood glucose and then pre-set the basal amount of insulin release and the large dose before meals as needed, so the level of medical knowledge of diabetic patients also puts forward high requirements. When the person with the pump takes a bath, exercises, etc. and needs to take off the insulin pump temporarily, if the pump is taken off for less than one hour, no special treatment can be done when the pump is taken off. When the pump is taken off for a longer period of time, the appropriate dose of insulin must be supplemented according to the specific situation. Insulin pump users generally need to change the insertion site of the injection device every 3 days, so as to prevent infection and obstruction at the insertion site. If you see any signs of redness or swelling at the injection site, you should find another injection site. It should be reminded that insulin pump is only a means of intensive treatment, and diabetic patients should also pay attention to comprehensive treatment in their daily life, and pay attention to diet therapy and exercise on the basis of using insulin pump, in order to receive good therapeutic effects with a multi-pronged approach.
