The following conditions require removal of the ovaries: i. Ovarian cancer, either primary or metastatic, requires removal of both ovaries. Cervical cancer. Endometrial cancer. For young patients, it is recommended to keep one ovary under further follow-up. IV. Ovarian pregnancy requires removal of ovaries. V. If the ovary is found to be a huge chocolate cyst and has the possibility of malignant transformation, the ovary needs to be removed. VI. If it is a benign ovarian tumor, ovarian cyst debulking surgery can be done to preserve normal ovarian tissue, but if the ovarian cyst is large and cannot be easily debulked, the affected ovary should be removed.
