With the globalization of aesthetic sensibilities, more and more patients are asking their surgeons to create a parallel or European shape for their blepharoplasty and inner-eye angle surgery. However, doing parallel and European style is actually very demanding for the surgeon, especially the treatment of the corner part of the eye, which requires the attending surgeon to be skilled in the anatomical features of the corner part of the eye, choose the most appropriate surgical plan, and handle every detail so as to successfully create a natural and stunning eyelid and corner shape. Unfortunately, there are numerous cases of failure after inner eye angle opening surgery, and the main problems are: 1. Too much skin removal at the corners of the eyes, resulting in severe defects in the vertical and horizontal directions; 2. Improper handling of the lower eyelid muscles and inner canthal ligaments, resulting in postoperative lower lid recession and excessive white exposure, appearing dull-eyed; and 3. Severe scarring in the inner eye angle area. In this case, the patient used a lower eyelid flap flip to repair the parallel corners of the eyes into a fan shape and reconstruct the Mongolian crease. The overall feeling of the patient’s eyes after the repair was significantly softer than before.
