The control of support resistance in orthodontic techniques is a critical issue in the success or failure of treatment of most orthodontic cases. Therefore, there is a consensus among orthodontists on the importance of orthodontic support. The correct design and good control of orthodontic support are necessary to achieve high quality orthodontic treatment by moving the teeth that should be moved and not moving the teeth that should not be moved. Insufficient support resistance is one of the main obstacles to the improvement of orthodontics. Since the end of the 20th century, with the development of material science, the orthodontic discipline has produced a breakthrough new technology based on the results of prosthodontics – micro-implant brace system. Unlike the previous orthodontic implant resisting systems, the micro-implant resisting system uses the hard bone as a direct bearer of the resisting force, and differs from the previous orthodontic implant resisting systems in that the implant does not rely on the bone bond for retention, but on the mechanical riveting force between the implant and the bone tissue. Moreover, these implants are temporary support devices that have to be removed after orthodontic treatment. The micro-implant placement area is flexible and can be placed either on the palatal side or on the labial, buccal or alveolar interval. The micro-implant can be placed manually with a tap, which reduces the osteotraumatic effects of heat on the interface during the implantation process. Since the implant bed is prepared without a pilot drill, the stability of the implant orientation prevents wobbling and displacement of the implant in the implant socket. The good initial stability of the implant provides for immediate force addition and allows for a shorter orthodontic treatment course. The simple and minimally invasive implant placement and removal procedure is easy for the orthodontist to perform independently and for the patient to accept. These titanium nail mini implants have a relatively low rate of loosening and fracture in application. They provide a stable support for all types of tooth movement and have obvious advantages. The miniature implant nails can be used in orthodontic clinics for (1) depression of incisors, absolute depression of upper anterior teeth to improve gingival smile, and absolute depression of lower anterior teeth to improve bony deep overbite; (2) severe crowding or severe arch protrusion, maximum internal retraction of anterior teeth to solve the problem of anterior protrusion; (3) when the anterior teeth are aligned or the posterior teeth have insufficient support; (4) upright or depressed molars and orthodontic heterotopic individual teeth; (6) anterior displacement of posterior teeth (6) Insufficient anterior tooth support when the posterior teeth move forward or when the posterior teeth move far from the center; (7) When the midline is significantly shifted; (8) When there is a lack of sufficient number of supporting teeth due to periodontal disease, missing teeth, or uncomfortable tooth position; (10) As lingual orthodontic support. Micro implant support can provide good support for tooth movement, which further expands the indications of orthodontics. The Department of Orthodontics of Nanjing Stomatological Hospital has now carried out the clinical application of this micro-implant bracing technology.
