Apply isotonic saline gastric lavage every night for three days before surgery for combined pyloric obstruction to reduce gastric mucosal edema and facilitate the healing of the anastomosis after surgery.  Second, postoperative guidance 1, pay attention to observe the wound dressing for blood and ooze, the color and amount of drainage fluid, and whether there are complaints of abdominal pain and abdominal distension. The purpose of cardiac monitor and precautions.  2.The purpose of performing gastrointestinal decompression is to suck out the accumulated gas and fluid in the stomach and intestines, reduce abdominal distension, and reduce the tension of the local anastomosis to reduce the occurrence of anastomotic fistula. Pain and discomfort in the throat will occur, pay attention to keep the mouth clean and moist, if necessary, can contain throat tablets to reduce discomfort, wait for the recovery of intestinal peristalsis, anal exhaust can be removed.  3. Intermittently clamp the urinary catheter and release urine once every 2 hours to perform bladder function training. Encourage to get out of bed early, usually the day after the removal of the urinary catheter on the second day after surgery, bedside activities to increase lung capacity, reduce pulmonary complications, improve systemic blood circulation, promote wound healing, prevent pressure sores, reduce lower limb venous thrombosis, facilitate the recovery of intestinal and bladder function, and reduce the occurrence of abdominal distention and urinary retention.  4. Encourage the patient to breathe deeply and cough effectively to expel sputum and prevent pulmonary complications.  5, fasting, fasting, the day of removal of the gastric tube on the day of removal first give a small amount of water or rice soup, 4-5 tablespoons each time, the second day into a half amount of liquid food, 50-80ml each time, 1-2 hours once; the third day into a full amount of liquid food, 100-150ml each time, 2-3 hours once; if there is no abdominal pain, bloating and other discomfort after eating, the fourth day can be into semi-liquid, to high protein, high vitamin, high calorie is good If there is no abdominal pain and bloating and other discomfort after eating, you can have semi-liquid food with high protein, high vitamin and high calories on the fourth day. Pay attention to a small number of meals (5-6 times a day), eat less milk, beans and other gas-producing foods, and avoid cold, spicy and other stimulating foods. Avoid eating too sweet and too hot fluids, and lie flat for 10-20 minutes after eating to prevent dumping syndrome.  3. Discharge guidance 1. Within one year after gastrectomy, the stomach volume is limited, so it is easy to have a small number of meals and eat a nutritious and easily digestible diet, and then slowly transition to a normal diet. Avoid raw, hard, spicy and irritating food, overeating, smoking and alcohol.  2, optimistic, keep a relaxed mood. Avoid overexertion.  3, need to take drugs, please strictly follow the instructions or follow medical advice, pay attention to the time, manner, dose and side effects of medication. Avoid taking drugs that are damaging to the gastric mucosa, such as aspirin, anti-inflammatory pain, corticosteroids, etc. Review regularly and consult for discomfort.
