Recently I will use my spare time to write something to let the majority of patients after the diagnosis of diabetes gives how to pay attention to, how to prevent, how to treat the program, so as to prevent the problem before it happens.  With the improvement of the national standard of living, the incidence of diabetes has almost doubled in recent years the number of patients suffering from the disease, which is a group that can not be ignored! The basic pathological change of diabetes is microangiopathy, and its corresponding retinal microangiopathy in the fundus is manifested as occlusion of microvessels, leading to ischemia, which is manifested in the fundus as early microangioma, hard exudation, soft exudation, and further development of neovascularization, eventually leading to hemorrhage of the retina, vitreous hemorrhage and retinal traction detachment. If we intervene with laser before the appearance of neovascularization, the patient is likely to maintain better vision.  There are many complications of diabetes such as liver and kidney insufficiency, lower limb skin ulceration, fundus hemorrhage, retinal detachment, blindness, etc. The root of all these pathologies is still high blood sugar, as elevated blood sugar can cause microvascular changes, which eventually lead to microvascular occlusion causing tissue ischemia. Therefore, blood sugar must be controlled, otherwise all the treatment will only be abandoned!  The author has recently encountered many patients in the clinic who ended up blind because they did not have timely or regular follow-ups. Even if we did surgery later, the recovery of vision was very poor. Recently, a patient with stage 6 diabetes mellitus underwent surgery to remove the lens and vitreous retinal glass, and it was found that all four retinal arteries emanating from the optic papilla were occluded, and a large neovascularized proliferating membrane grew in the optic papilla (fundus). We took down the surgery, but we can imagine that his vision will not be good, because all the blood vessels pouring into the retina are occluded, can it function well? If the patient wants to have regular follow-ups for 2-3 years, the surgery or laser will not delay his condition so badly. For diabetic friends “regular follow-up” is something you must do!  In clinical practice, I have met many patients who have been treated in a timely manner. When they reached the proliferative stage of diabetic retinopathy, we gave timely laser treatment and the patient’s vision not only did not decline but also improved. However, we have encountered quite a few patients in the clinic who do not take themselves seriously or are afraid of treatment, which eventually leads to lost treatment opportunities. A diabetic patient once came to see me 3 times, both eyes to the stage of diabetes 5-6 is afraid of surgery is still afraid to do surgery, still suffering from blindness, suffering from the darkness of pain. Recently, I have done 2 cases of patients with very serious glycoretinopathy, they are also very determined to come, after examination of the patient’s retinal function has been very poor, we gave the surgery effect is naturally very poor, doctors suffer naturally not to say, but the surgery effect is so poor, I myself also very distressed. Why did you come so late? Spent money, suffered, poor results! It’s all because the treatment time was missed!  Four, do not believe that who has a good way to treat fundus bleeding Diabetic fundus bleeding is due to diabetic changes caused by vascular occlusion resulting in tissue local ischemia caused by compensatory vascular hyperplasia, the biggest difference between this neovascularization and normal blood vessels is its unsound, easy to bleed, so to treat this bleeding is the fundamental is to eliminate this bad neovascularization. And the best way to eliminate this neovascularization is fundus laser, which is recognized by everyone.  Some people say that there is a good way to treat this bleeding, but in fact this is only treating the symptoms, not the root cause, because there is no medicine (hemostatic or blood-activating medicine) that can eliminate the neovascularization. After this treatment, even if the patient feels that his vision has improved, he may bleed again soon, because this treatment only treats the symptoms. I have encountered many patients with recurrent bleeding in the clinic, but eventually chose vitreous surgery plus fundus laser treatment!
