The INTERTAN intramedullary nail is a new generation of intramedullary nails designed specifically for proximal femur fractures by smith&nephew in July 2006. It was first reported by Ruecker in JOURNAL OF ORTHOPAEDIC. 1. Advantages Wide range of indications, basically can be used for all femoral fractures. For example, femoral neck fractures, cis and intertrochanteric fractures, subtrochanteric fractures, etc. However, its biggest advantage is the use of femoral multi-segment fractures, e.g.: femoral neck fractures combined with diaphyseal fractures. For the fixation of A2 and A3 fractures, the reconstruction nail has the advantages of firm fixation and anti-rotation than the joint plate. (1) The combination of interlocking nails has a high degree of stability, avoids the “z-effect” of traditional reconstruction nails, resists rotation, prevents cutting out, and has a significant pressure effect (2) The trapezoidal cross-section of the proximal end is the design principle of the joint prosthesis stem, which enhances stability and mechanical advantages, and the smaller proximal end maximizes the preservation of normal bone and surrounding soft tissue. Preserve the normal bone and surrounding soft tissue. (3) The distal end adopts a hairpin-like bifurcation design to reduce stress concentration, avoid fractures around the distal end, and reduce the occurrence of pain. (4) The design with 12° anterior tilt angle is more in line with the anatomical characteristics. (5) Pre-positioned hollow stabilization screws can be locked when necessary to eliminate excessive postoperative sliding. 2. Shortcomings (1) The position requirement is relatively high. The preoperative position is an important part of the success of the operation and the length of the operation. This technique is not suitable for obese patients. (2) The cost of internal fixation is relatively high and some patient groups are lost. (3) The surgical technique is demanding, especially the selection of the nail entry point, which requires repeated practice and experience. (4) It is a new type of intramedullary fixation, and its long-term efficacy is still being explored at home and abroad. Zhu Shulang, Department of Orthopedics, Affiliated Hospital of Jiujiang College, an 82-year-old male patient with left femoral neck fracture, operated after one month of treatment for combined pulmonary and cardiac disorders, under epidural anesthesia, in fact, the position is not so complicated as described above, the price is 15,000, the operation time is 40 minutes, and the patient can exercise in this way on the day after the operation.
