Minimally invasive surgical approach to myomectomy

  (1) Laparoscopic myomectomy The advantages of laparoscopic surgery: ① Less intraoperative bleeding, less postoperative pelvic adhesions than open surgery.  ②Faster postoperative recovery, 6 hours after surgery can be down to the ground, and 2 weeks later can return to the preoperative activity.  ③Aesthetic effect of abdominal wall, only 3 small wounds of 0.5-1 cm on the abdominal wall.  Indications for surgery: ①Indications for myomectomy, no contraindication to laparoscopic surgery.  ②Single myoma diameter ≤10cm, and preferably no more than 3 multiple intermural myomas.  (2) Hysteroscopic myomectomy Indications for surgery: ①Submucosal fibroids of maximum diameter ≤5cm, and myoma with a clitoris is not limited in size.  ②Small asymptomatic submucosal fibroids with fertility requirements.  Transvaginal hysteroscopic surgery is extremely minimally invasive and returns to preoperative state after 2-4 hours postoperatively.  (3) Yin-type myomectomy Indications for surgery: ①No pelvic adhesions and a relaxed vagina.  Preferably single fibroids with a diameter of ≤10cm.
