Gastric cancer is a malignant tumor occurring in the epithelial tissue of the stomach, and it is the most common malignant tumor in the world (including China). In our country, its incidence ranks the first among all malignant tumors and the mortality rate remains high, which shows that gastric cancer seriously threatens the health of our people. In recent years, it is believed that salt-cured food, geographical environment (cold areas), geological water system (lack of trace elements), nitrate intake or asbestos inhalation, poor dietary behavior, host factors (male, type A blood, suffering from chronic atrophic gastritis, gastric ulcer, gastric polyps and other diseases), lack of refrigeration conditions, and low regional socioeconomic level are all related to the occurrence of gastric cancer. From the medical history, patients with gastric cancer mostly have unexplained loss of appetite, persistent pain and discomfort in the upper abdomen, fullness, anorexia, nausea, black stool, and further development of ascites, jaundice, obvious emaciation, swelling, anemia and cachexia; or age >40 years old, history of chronic gastric disease with recent symptoms significantly aggravated, or had most of the stomach removed and recently developed upper gastrointestinal symptoms. Symptoms of different stages of the disease have different characteristics: early symptoms are not obvious or non-specific gastrointestinal symptoms or similar to typical peptic ulcer symptoms such as epigastric fullness or vague pain, acidity, belching, nausea, loss of appetite, etc.; in the progressive stage, there is biting pain in the stomach area, which is not obviously related to eating, as well as epigastric pain, heaviness, vomiting, diarrhea, emaciation, anemia, edema, and fever. The symptoms of different parts of the stomach have different characteristics: pancreatic cancer is characterized by subxiphoid discomfort, obstruction in eating or difficulty in swallowing; cancer of gastric body has late appearance of pain; cancer of ulcerated type is most common on the small curvature of gastric sinus, so the symptoms of epigastric pain appear earlier; cancer of fundus and subcardia often has no obvious symptoms until the tumor is huge and necrosis breaks down, leading to bleeding in upper gastrointestinal tract, or the tumor infiltrates to the cardia and causes difficulty in swallowing before attention is paid to it. Chinese medicine treatment is an important aspect of comprehensive treatment of gastric cancer and can be used throughout the whole process of gastric cancer treatment. Long-term clinical practice has proved that TCM has its unique advantages in the treatment of gastric cancer. Chinese medicine treatment is guided by holistic view and based on evidence-based treatment, emphasizing the combination of evidence-based and disease-based treatment, local treatment and overall treatment, and the combination of positive treatment and elimination of evil. For different treatment purposes of patients, the focus of TCM treatment is different. For patients whose tumors have not been removed, the focus is on effectively improving the immune function of the body, improving clinical symptoms, improving quality of life and prolonging survival; for patients whose tumors have been removed, preventing their recurrence or metastasis and promoting the body’s postoperative qi and blood recovery; for patients who have undergone chemotherapy and radiotherapy, the aim is to reduce chemotherapy resistance, increase patients’ sensitivity to treatment and relieve toxic side effects after treatment. 1.Chinese herbal medicine with surgery treatment. It means that before the surgery, the patient’s qi, blood, yin and yang, and the function of the internal organs are regulated in order to restore the state of “yin and yang” to the maximum extent, which is conducive to ensuring the smooth operation and reducing postoperative complications. The main formula is Si Jun Zi Tang, Si Wu Tang, Ba Zhen Tang, Shi Quan Da Teng Tang, Bao Yuan Tang, Liu Wei Di Huang Tang, etc. After surgery, by cultivating qi and blood, regulating the spleen and stomach, and regulating immunity, we can alleviate the damage caused by surgery and postoperative complications, promote the body’s recovery, and create conditions for the implementation of radiotherapy. Within 1 month after surgery, the main treatment is to strengthen the spleen, harmonize the stomach and lower the rebellion, promote the flow of qi and dampness, and nourish yin and produce fluid, and the formula is selected from Xiang Sha Liu Jun Zi Tang, Er Chen Tang, Ling Gui Zhu Gan Tang, Sha Shen Mai Dong Tang, etc. After 1 month, the physical strength gradually recovers, anti-cancer drugs and blood activating drugs can be added to reduce recurrence and 2, Chinese medicine with chemotherapy. It can be divided into two stages, one is the adjuvant treatment of Chinese medicine during chemotherapy, mainly using the function of supporting and regulating Chinese medicine to reduce the toxic side effects of chemotherapy drugs on the body, stabilize the internal environment of patients, improve the immune function and the sensitizing effect of certain drugs to improve the effect of chemotherapy. For example, for those with bone marrow suppression, Astragalus, Radix Codonopsis, Radix Angelicae Sinensis, Radix Rehmanniae, Rhizoma Polygonati, Rhizoma Polygonati and Rhizoma Polygonati; for those with gastrointestinal reactions, Radix Ginseng and Rhizoma Atractylodis soup, Er Chen Tang and Xiang Sha Liu Jun Zi Tang can be used according to the evidence; Ten Complete Tonic soup has a potent detoxifying effect, and blood activating drugs such as Radix Angelicae Sinensis, Rhizoma Hong Hua and Rhizoma Chuanxiong can promote the action of chemotherapeutic drugs on the lesions and play a sensitizing effect. Secondly, between the end of each chemotherapy course and the beginning of the next course, the application of Chinese medicine is to help dispel evil and consolidate the effect of chemotherapy, using evidence-based treatment, on the basis of nourishing the blood, strengthening the spleen, benefiting the kidneys, together with clearing heat and detoxifying, activating blood circulation and removing blood stasis, softening and dispersing hard nodules to strengthen the anti-tumor effect. 3. Chinese medicine with radiotherapy. As Chinese medicine believes that the damage of radiation to the body is heat toxicity consumes qi and injures yin and damages fluids. Treatment is to benefit qi and nourish yin, clear heat and detoxification, strengthen the spleen and stomach, and nourish the liver and kidney. Particular attention should be paid to the protection of the spleen and stomach. The medicine is Astragalus, Radix et Rhizoma, Radix et Rhizoma, Radix et Rhizoma, Radix et Rhizoma, Radix et Rhizoma, Radix et Rhizoma, Radix et Rhizoma, Radix et Rhizoma, Radix et Rhizoma, Radix et Rhizoma, Radix et Rhizoma, Radix et Rhizoma, Radix et Rhizoma, Radix et Rhizoma. Chinese medicine should be taken 3-4 days before radiotherapy until six months after the end of radiotherapy to ensure the successful completion of radiotherapy and minimize the side effects. 4.The treatment of middle and late stage gastric cancer starts from spleen and kidney, and cultivates the yuan and replenishes the deficiency. Patients with middle and late stage gastric cancer who cannot be removed surgically or have no chance of chemotherapy after surgery have their vital energy greatly injured, therefore, they should supplement the spleen and kidney to cultivate the vital energy, nourish the internal organs and mobilize the body’s ability to fight cancer. The herbs include Radix et Rhizoma Ginseng, Atractylodes Macrocephala, Fructus Lycii, Radix et Rhizoma Chasteberry, Radix et Rhizoma Rabbitii and Radix et Rhizoma Bone Marrow. 5.Chinese medicine control tumor recurrence and metastasis. The control of recurrence and metastasis after surgery and radiotherapy has become a hot spot of research in Chinese medicine for tumor treatment. Tumor recurrence and metastasis are characterized by internal deficiency of positive energy and internal obstruction of blood stasis, and treatment is based on strengthening the spleen and benefiting the energy, nourishing the blood and harmonizing the middle, supplemented by activating blood circulation and removing blood stasis. The medicine uses Astragalus, Coix seed, turtle nail, angelica, poria, snake tongue herb, leech, centipede, etc.
