What to do about atherosclerosis in both lower extremities

  Double lower extremity vascular sclerosis refers to the narrowing or even occlusion of the arterial lumen due to the enlargement or rupture of atherosclerotic plaques in the lower extremity arteries. Mainly conservative treatment, taking antiplatelet drugs and vasodilator drugs, serious cases take interventional treatment.  1.Non-surgical treatment: lower blood lipids, stabilize arterial plaque, improve hypercoagulation, dilate blood vessels and promote collateral circulation. Anti-platelet drugs, such as: aspirin, clopidogrel, etc. Vasodilators, such as prostaglandins. Patients who are asymptomatic, have mild symptoms, or are not candidates for surgical intervention can be treated with non-surgical methods. Of course, non-surgical treatment is also an adjunct to the surgical/interventional treatment described below.  2.Surgical/interventional treatment: The aim is to reconstruct arterial access through surgical or endovascular treatment methods.  (1) Percutaneous endoluminal angioplasty: Percutaneous puncture to the stenotic segment of the artery is used to inflate the balloon with appropriate pressure to enlarge the diseased lumen and restore blood flow. Combined with the application of stents, the long-term patency rate can be improved. The use of endoluminal treatment for stenosis and occlusive lesions of the iliac artery has shown positive results. Currently, it is also used to treat single or even multiple stenoses or occlusions of the femoral artery and its distal arteries, and in most cases, good limb-saving results can be achieved.  (2) Endothelial debridement: The debridement of thickened intima, atheromatous plaque and secondary thrombus in the diseased artery is mainly used for short-term occlusive lesions of the iliofemoral artery.  (3) Bypass diversion (bypass): using an autologous vein or artificial vessel to make a bypass diversion between the proximal and distal ends of the occluded segment.  (4) Other: large omental graft, revascularization therapy, etc.
