Large cerebral infarction coma can be treated by intravenous thrombolysis or surgery. 1. Intravenous thrombolysis: acute cerebral infarction can be treated with acute thrombolytic drugs recombinant tissue-type fibrinogen activator, such as alteplase and reteplase to dissolve the embolus, so as to make the blood vessels recanalized and let ischemic brain tissues get blood supply. 2. Surgical treatment: commonly used surgical methods include carotid endarterectomy and carotid angioplasty, debridement and decompression. (1) Carotid endarterectomy: Separate the carotid artery’s lining from its outer lining through surgery, and remove part of the lining to alleviate the lack of blood supply. (2) Carotid angioplasty: for patients who cannot undergo carotid endarterectomy. (3) Debridement decompression: applicable to patients with large cerebral infarction and obvious edema, debridement decompression can prevent cerebral hernia formation.
