What is the purpose of the “ambulatory electrocardiogram” test?

  The ECG, commonly known as the “backpack”, is a common examination tool in cardiology, consisting of seven electrodes and a box. In our daily clinical work, we often encounter patients carrying “ECG” on their backs and asking doctors what to do next. So what are the specific uses of the ECG? How can we cooperate with the doctor when performing an ECG so that we can make good use of the test and obtain more relevant information?  Generally speaking, an ECG is commonly used in hospitals to record ECG activity over a 24-hour period. Compared with the general ECG, the ECG can record up to 100,000 ECG signals continuously within 24 hours, which can improve the detection rate of non-sustained arrhythmias, especially transient arrhythmias and transient episodes of myocardial ischemia.  It has the following main functions: 1. It can record the total heart rate for a day, and can detect slow arrhythmias such as sinus bradycardia and pathological sinus node syndrome.  2.It can record potential cardiac arrest, such as sinus arrest and atrioventricular block.  3.The severity of certain arrhythmias can be assessed, such as recording the number of occurrences of arrhythmias such as ventricular premature beats and atrial premature beats within 24 hours.  4.Monitoring tachyarrhythmias and capturing occult arrhythmias: such as paroxysmal supraventricular tachycardia, short paroxysmal atrial tachycardia, ventricular tachycardia, paroxysmal atrial fibrillation, etc.  5. It can also capture episodes of myocardial ischemia and provide a basis for the diagnosis of coronary artery disease.  What do we need to pay attention to when performing “ambulatory electrocardiogram” test?  1.First of all, follow the doctor’s advice: decide whether you need to adjust the medication, such as the assessment of premature ventricular beats, you need to stop the anti-arrhythmia.  2.It is better to move than to be still: after wearing the recorder, the daily life should be the same as before wearing it, and the examinees should do moderate exercise. According to the condition and the purpose of the examination, inpatients can walk slowly, up and down the stairs, etc.; suspected angina pectoris patients can choose more intense exercise that may trigger the onset of the disease, so as to observe the relationship between the amount of exercise and myocardial ischemia, arrhythmia, for the doctor’s diagnostic reference. However, it is necessary to avoid touching the electrode when exercising in order not to interfere with the image acquisition.  3, the skin should be dry: electrodes are attached to the forehead skin through the wire connected to the recorder, if the skin is wet, the contact between the electrodes and the skin is not good, or even cause the electrodes to fall off, the subject will have to redo. Therefore, the examination day need to avoid sweating, can not take a shower, for the clean people is a “difficult” day.  4, it is appropriate to record: remember to check the day of discomfort and exercise time detailed registration, you can find out this time period of the ECG to see if there are changes, to provide a reliable basis for the doctor’s diagnosis and treatment.
