According to Chinese medicine, the etiology and pathogenesis of headache are complex. After analyzing the etiology and pathogenesis of more than 2000 cases of migraine patients accumulated in the clinic, we believe that the etiology and pathogenesis of migraine have the following characteristics: (l) windy head: the attack of migraine is characterized by sudden onset and sudden termination of headache. This is because wind is a Yang evil and its nature is light, “Above the top height, only the wind can reach”, and the Jin Kui Yi says: “Migraine is caused by the wind evil visiting the upper Yang meridian, and its meridian is partial deficiency, and the evil gas comes together on one side, and the pain is even in the forehead corner for a long time, so it is called migraine.” Therefore, windy head is an important pathogen of migraine. (2) Multiple deficiency in the head: The head is the meeting of all the yang, after the sun travels to the head, the lesser ground travels to the side of the head, the sun and the lesser yang are the two meridians of deficiency of qi and blood, because of their meridian deficiency, and committing the wind evil, the two deficiencies get each other and form headache. The deficiency of the head is an important precondition for the occurrence of migraine. (3) Multiple stasis in the head: The nature of migraine pain is throbbing, stabbing, swelling, etc. The pain of long-standing disease is mostly in the position of Shao Yang, and it is mostly seen with stringent pulse and stasis on the tongue. There are two reasons for this: the position of the head is prone to stasis of blood due to smashing; the head is covered by the two meridians of liver and gall bladder, the bile meridian travels to the side of the head and the liver meridian travels to the top of the head, both meridians are responsible for draining, and draining out of balance can produce stasis of blood; long-standing disease entering the luo can also produce stasis of blood, therefore, multiple stasis of blood in the head is another important pathogenic mechanism of migraine. (4) Dampness in the head: Migraine patients are often accompanied by dizziness and poor diet, and swelling can be seen in premenstrual migraine attacks in women. This kind of phlegm-damp and watery flooding is because the qi of sympathetic Yin carries the qi of spleen deficiency and phlegm-dampness upward. The combination of wind, deficiency, stasis and dampness in the head leads to the development of migraine. For wind-cold type migraine, combining Peach Red Four Objects Tang with Ephedra with Pineal Seeds and Pineal Pains Tang should be used; for wind-heat type migraine, combining Peach Red Four Objects Tang with Scutellaria Baicalin, Chai Hu and Sheng Shi Fa; for blood stasis type migraine, Peach Red Four Objects Tang with Chicken Blood Vine and Yan Hu can be used; for phlegm-damp type migraine, combining Peach Red Four Objects Tang with Wu Ling San or Er Chen Tang can be given; for deficiency, Qi deficiency is the main reason for combining Four Objects Tang with Peach Red Four Objects Tang, and for kidney deficiency, combining Qiju Dihuang Tang with In cases of kidney deficiency, Qi Ju Di Huang Tang is combined with Si Wu Tang. In the treatment, the following points should be noted: ① it is a chronic disease with long treatment time, even if the identification is accurate, at least one month of continuous medication; ② all kinds of headaches can transform into each other and can be combined, it must be noted that all kinds of headaches can be trapped in stasis; ③ long-term treatment pay attention to qi deficiency, patients who take a lot of blood activating and wind removing medicine for a long time, most patients will have qi deficiency symptoms, attention must be paid to replenish qi.
