Gastroparesis is a gastrointestinal condition in which pain occurs frequently in the upper abdomen. This evidence is mostly seen in acute and chronic gastritis, gastric and duodenal ulcers, gastric spasm and other diseases. 1. Diagnosis 1.1 Clinical manifestations: Gastroparesis can be clinically divided into cold evil offending the stomach, dietary stagnation, liver qi offending the stomach, spleen and stomach deficiency cold and blood stasis. From the process of development, no matter what kind of gastroparesis is not thoroughly treated and does not heal for a long time, stasis of blood can be formed. 1.1.1 Gastroparesis caused by cold evil: violent epigastric pain, cold stomach with warmth, pain relieved by heat, thirsty mouth, hot drinks, white fur, tight pulse. 1.1.2 Stagnation in the diet: distention and fullness in the stomach and epigastrium, even pain, belching and swallowing acid, or vomiting undigested food, pain decreases after vomiting, or unpleasant stools, thick and greasy coating, slippery pulse. 1.1.3 Liver qi offending the stomach: gastric distention and fullness, painful attack, pain in the epigastrium and hypochondrium, belching, unpleasant stool, painful due to emotional factors, thin white fur, and stringent pulse. 1.1.4 Stomach and spleen deficiency cold: vague stomach pain, vomiting water, warmth and pressure, reduced appetite, fatigue, loose stools, pale white tongue, weak or sunken pulse. 2. Evidence 2.1 External cold, which offends the stomach, or over-eating cold, which accumulates cold in the stomach, can cause stomach pain due to cold, especially for those who have a weak spleen and stomach, who are more likely to feel cold and develop pain. In addition, the accumulation of worms also leads to gastroparesis. 2.2 Depression and anger hurt the liver, and liver qi loses its drainage, causing gastric and epigastric pain. Stagnation of liver qi can turn into fire, and fire can injure yin, which can aggravate pain or make the disease lingering. 2.3 Weakness of kidney yang, or overwork, hunger and satiety can damage the spleen and stomach, making the middle qi weak and cold and painful. 3. Treatment 3.1 Principle of treatment: “Regulating qi and relieving pain” is the main treatment. 3.2 Basic treatment: 3.2.1 Abdominal operation 3.2.1.1 Acupuncture points: Zhonggui, Qihai, Tianshu, Shusanli, Shuangjing. 3.2.1.2 Operation: The patient lies on his back, the doctor sits on his right side, firstly, use a slight one-finger Zen pushing method from the middle epigastric to the qihai, treating 3-5 times round trip; then use palm rubbing method to operate on the stomach and abdomen for 5 minutes to make the heat penetrate into the stomach and internal organs; point and rub the middle epigastric, qihai and tianshu to the extent of soreness and swelling; then let the patient lie on his side facing the doctor, push and press the stomach and abdomen with the palm, while the other hand presses and rubs the foot three li and shoulder well. The other hand presses and rubs the foot and shoulder points. 3.2.2 Back operation: 3.2.2.1 Acupuncture points: the first lateral line of the bladder meridian on the back, with emphasis on the liver, spleen, stomach, and Sanjiao yu. 3.2.2.2 Operation: with the patient in prone position, use kneading to operate the bladder meridian on both sides of the back from top to bottom 3–5 times; then apply heavier pressure to the liver, spleen, stomach and Sanjiao yu, to the extent of soreness and swelling; then knead the spine 5–6 times. 3.2.3 Upper extremity and hypochondrium operations: 3.2.3.1 Acupuncture points: Hand Sanli, Neiguan, and Hegu. 3.2.3.2 Use the pressing and rubbing method to make strong stimulation at the hand three li, nei guan and hegu points; then rub the upper extremities; then rub their two hypochondrium, from top to bottom several times round trip. 3.3 Identification of evidence and addition and reduction 3.3.1 Cold evil offending the stomach: 3.3.1.1 Matching: warming the middle and dispersing cold. 3.3.1.2 Operation: Stimulate Hegu, Spleen Yu and Stomach Yu with heavier pointing and pressing, and then treat with lateral rubbing on the back to penetrate heat. 3.3.2 Food stagnation: 3.3.2.1 Coordination: elimination and neutralization. 3.3.2.2 Operation: knead the abdomen in a clockwise direction, focusing on the central epigastric and tianshu points, for about 5 minutes; press the spleen yu, stomach yu, large intestine yu, and foot sanli with heavy hands. 3.3.3 Liver qi offending the stomach: 3.3.3.1 with: dredging the liver and regulating qi. 3.3.3.2 Operation: treat with gentle one-finger zen pushing or kneading from the tiantu down to the middle cavity, focusing on the danzhong point; then gently press and knead the zhangmen and shimen points on both sides; then press and knead the liver yu, bile yu, diaphragm yu, and taichong points with heavier techniques. 3.3.4 Spleen and stomach deficiency cold: 3.3.4.1 Matching: warming and tonifying spleen yang. 3.3.4.2 Operation: focus treatment on Qihai, Guanyuan, Sansili, and Sanyinjiao with gentle pressing and rubbing, with appropriate extension of treatment time at Qihai point; direct rubbing of the dorsal Governor; horizontal rubbing of the Kidney Yu and the Gate of Life, to penetrate heat. 3.3.5 For those with severe pain: first stimulate Diaphragmatic Yu, Spleen Yu, Stomach Yu, Hegu, Neiguan and Liangqiu points with heavier pressing and kneading, and then treat them with evidence after the pain is relieved. This disease is mostly caused by acute and chronic gastritis, gastric and duodenal ulcer in modern medicine, and generally has a long duration, so the treatment takes a long time. The treatment should be persistent and should not be abandoned halfway. Patients with bleeding gastric and duodenal ulcers should not be treated by manipulation. Patients should be instructed to live a regular life, pay attention to diet regulation, be cheerful and not to be overly tired.
