Dizziness is a symptom often seen in daily life and in clinical practice. Most people have experienced dizziness at one time or another in their lives. Because of its prevalence, the number of clinical consultations is relatively high. Recently, because of the popularization of scientific knowledge and the use of the Internet, people have more or less understood some medical knowledge. After the symptoms of dizziness, many patients first think of cervical spine problems, hoping to do cervical spine examination or corresponding treatment. Is this really the case? Today we will talk about the problem of dizziness. Dizziness is a purely symptomatic thing, and dizziness is not the same as halo. The symptom of dizziness can be divided into two main types, one is called dizziness and the other is called vertigo, and their clinical manifestations are different. First of all, let’s talk about vertigo, vertigo’s biggest feature is that the patient has the illusion of visual space, which simply means that the patient has the feeling that the sky is spinning. Some patients feel that the things around them are spinning, while others feel that they are spinning. Regardless of the patient’s feeling, the common characteristic is that there is a relative displacement, which is called vertigo. Another concept is called dizziness, this part of the patient does not have the feeling of heavenly rotation, it is only the feeling of the head of the dizziness, some people will describe my head dizziness all day and night are not clear, in addition can be accompanied by, for example, memory loss, cervical spine stiffness, shoulder muscle stiffness, insomnia or accompanied by mood disorders, and so on. Therefore, we say that whether there is a sense of rotation is the key to identify vertigo and dizziness, and also the key to make a localized diagnosis. What are the causes of these two different kinds of dizziness? First of all vertigo we also call systemic vertigo or true vertigo. So what does this system refer to? It refers to the human vestibular system, which is a set of organs that exist in the inner ear that governs the body’s sense of balance, and it mainly consists of the vestibular nerve center and the vestibular apparatus. If the patient has a dizzy spell, this is called vertigo, which is related to the vestibular system. If you feel dizzy, like wearing a hat, all day is not clear, combined with fatigue, insomnia, cervical stiffness, this is called dizziness or dizziness, not vertigo, its lesion location is not in the vestibular, but with the cerebral vessels, cervical vertebrae, or systemic status related to, for example, anemia, mood disorders, and so on. How should we choose the right specialty to consult after the symptom of dizziness appears? If the symptoms are vertigo with a feeling of spinning, we suggest that the first department to consult is otorhinolaryngology, with vestibular function examination if possible, and clinical examination to exclude otoliths in the inner ear. Therefore, ENT should be the first department to identify vertigo. If the patient does not have the sensation of spinning in the sky and the initial judgment is dizziness, how should the patient consult the doctor? First of all, we can see the neurology department, except from the neurological point of view, such as cerebral ischemia and so on, these non-vestibular etiology. In addition, there are some basic tests, such as routine blood tests, blood biochemistry tests, thyroid function tests, etc., which can be done to improve the condition. So to summarize, dizziness is not the same as halo, and the examination and treatment of different halos are also different, so we can grasp this basic concept to further down the examination and treatment.
