Diagnosis and Chinese medicine treatment of allergic rhinitis

  Diagnostic criteria for perennial allergic rhinitis
  1. Scoring conditions.
  (1) Perennial onset, with three main clinical manifestations: sneezing (more than 3 in a row each time), runny nose and swollen nasal mucosa, with a cumulative number of onset days exceeding 6 months in a year and a cumulative onset time exceeding 0.5 hours in a day.
  (2) The duration of the disease is at least 1 year.
  2. Scoring criteria.
  There are clear clues of inhalant allergens, personal and/or family history of allergic diseases, and typical symptoms and signs during the attack period, each of which is scored 1 point, for a total of 3 points. Positive allergen skin test reactions, at least one of which was (++) or (+++) or more; positive specific IGE antibody test or positive allergen nasal excitation test, and consistent with the skin test and medical history, each scored 2 points, for a total of 4 points. Positive nasal secretion smear for eosinophilic strong cells scored 1 point each (or) positive nasal mucosal scrapings for mast cells (basophils).
  A score of 6-8 is diagnosed as perennial allergic rhinitis, 3-5 as suspected allergic rhinitis, and 0-2 as possible non-allergic rhinitis.
  Hay fever diagnostic criteria
  1, seasonal onset, the onset of the season is basically the same every year, and it is consistent with the pollination period of allergenic pollen, at least two years in the same season.
  2.There are typical clinical symptoms and signs during the attack period.
  3. Positive eosinophils in nasal secretions (and/or conjunctival scrapings) or positive mast cells (basophils) in nasal mucosal scrapings during the attack period.
  4. Positive skin test for pollen allergens, at least one of which is (++) or (+++) or more, or positive allergen nasal excitation test or positive conjunctival test.
  Protocols for the treatment of symptoms
  (i) General program.
  1.Acupuncture.
  First acupuncture: milli-needle: Yingxiang, Yin Tang, Baihui, Lijiu or Fengchi, Feosanli, Hegu, alternating between the two groups of points.
  Second moxibustion: direct moxibustion: Yingxiang, Baihui, Yin Tang, 5-7 strong / point; drug moxibustion: Lung Yu, Spleen Yu, Kidney Yu or Fengmen, Stomach Yu, Zhiguro, two groups of alternating, geng day moxibustion or three volt moxibustion, every 10 days / time.
  Triple consolidation.
  (1) Intradermal acupuncture: Lung, Spleen, Kidney or Fengmen, Stomach and Zhiguan.

  (B) Identification and reduction of symptoms
  1. Lung deficiency and cold sensation type: main symptoms: paroxysmal nasal itching and continuous sneezing. Concurrent symptoms: malignant wind and cold, white face, shortness of breath, cough, white sputum. Thin white tongue coating and floating pulse.
  (1) Acupuncture protocol: the same as the general protocol.
  (2) Chinese herbal soup: Ma Huang Tang.
  2. Spleen qi weakness: main symptom: itchy nose with continuous sneezing. Concurrent symptoms: high amount of clear snot, weakness of the limbs, loose stools. Pale red nasal mucosa. Pale tongue, white coating and weak pulse.
  (1) Acupuncture: add moxibustion to the general protocol: take Spleen Yu and Foot San Li.
  (2) Chinese herbal soup: Chen Xia Liu Jun Tang with reduction
  3. Deficiency of kidney yang: itchy nose, nasal congestion, more sneezing. Concurrent symptoms: cold fear, cold extremities, clear and long urine, loose stools. Pale white nasal mucosa and edema of the turbinates. Pale tongue, white coating, sunken pulse.
  (1) Acupuncture and moxibustion protocol: direct moxibustion: general protocol plus moxibustion of kidney yu and vital gate.
  (2) Chinese herbal tonics: Right Return Pill with addition and reduction.
  (3) Treatment of symptoms
  1. Nasal congestion is the main problem.
  (1) milli-needle: Baihui, Su s, Fengchi.
  Can be combined with the choice of abdominal acupuncture: inducing Qi to return to the Yuan (deep stab), Shang Qu (medium stab), Yin Du (shallow stab), upper wind-damp point (shallow stab) acupuncture flat tonic and flat diarrhea, once a day.
  (2) moxibustion: Baihui, pressure moxibustion, 3-5 strokes.

Indications: deficiency of kidney yang: lumbar pain and weakness of the feet, cold sensation below the half-body, constriction of the abdomen, unfavorable urination, or excessive urination, especially at night, impotence and premature ejaculation. Usage and dosage: Take orally, 10 capsules each time, 3 times daily.
  3, tonic zhong yi qi pills: composition: astragalus, ginseng, ascending marijuana, Chai Hu, Chen Pi, angelica, atractylodes, licorice. Effects: Tonifying the middle and benefiting the qi, raising the yang and lifting the trap. Indications: Spleen does not rise and clear the evidence. Effects: Helping Yang and benefit Qi, dispersing cold and relieving symptoms. Indications: Weak yang energy, cold and wind-cold symptoms. Headache and fever, heavy cold and light heat, no sweat, cold limbs, lethargy, pale face, low language, etc. Usage and dosage: Take orally, 1 pill each time, 2 times daily.
  (iv) Required laboratory tests
  Blood routine, allergen test, nasal secretion smear (acidophilic (alkaline) granulocytes), nasal secretion IgE, serum IgE, rhinoscopy, etc.
