Diagnostic criteria for neurosis and obsessive-compulsive disorder

  Neurosis is a group of psychiatric disorders that manifest mainly as anxiety, depression, fear, obsessive-compulsive, hypochondriac symptoms, or neurotic symptoms. The disorder has a personality basis, and its onset is often influenced by psychosocial (environmental) factors. The symptoms are not based on a verifiable organic lesion and are not proportional to the patient’s reality, but the patient feels distressed and powerless about the presence of symptoms, with complete or largely complete self-awareness and a prolonged course. Various neurological symptoms or their combination can be seen in infectious, toxic, visceral, endocrine or metabolic and organic brain diseases, called neurological-like syndrome.
  [Symptom criteria] At least 1 of the following.
  ① fear.
  ② obsessive-compulsive symptoms.
  ③ panic attacks.
  ④ anxiety.
  ⑤ somatoform symptoms.
  (vi) somatization symptoms.
  (vii) hypochondriac symptoms.
  ⑧Neurasthenia symptoms.
  [Severity criteria] Impaired social functioning or inescapable mental distress, prompting them to actively seek medical treatment.
  Criteria for course of illness】Meeting the criteria for symptoms has been at least 3 months, panic disorder is otherwise specified.
  Exclusion criteria】Exclude organic mental disorders, psychoactive and non-addictive substance-induced mental disorders, various psychotic disorders, such as schizophrenia, paranoid psychosis, and mood disorders, etc.
  OCD refers to a neurological disorder characterized by obsessive-compulsive symptoms, which is characterized by the coexistence of conscious self-compulsion and counter-compulsion. The patient is also aware of the abnormality of the symptoms, but cannot get rid of them. Those with prolonged illness may have ritual actions as the main cause of mental suffering, but social function is severely impaired.
  [Symptom Criteria
  (1) Meet the diagnostic criteria of neurosis with predominantly obsessive-compulsive symptoms, with at least 1 of the following.
  (1) predominantly compulsive thoughts, including obsessive-compulsive ideas, memories or representations, obsessive-compulsive oppositional ideas, exhaustive thoughts, fear of losing self-control, etc.
  ② predominantly compulsive behaviors (actions), including repeated washing, checking, examining, or questioning, etc.
  (iii) A mixed form of the above.
  (2) The patient claims that the obsessive-compulsive symptoms originate from within him or herself and are not imposed by others or external influences.
  (3) Recurrent obsessive-compulsive symptoms, which the patient perceives as meaningless and feels unpleasant or even painful, and therefore tries to resist, but cannot be effective.
  Severity Criteria】 Impaired social function.
  Criteria for the duration of the disease】Meets the criteria for symptoms for at least 3 months.
  【Exclusion criteria
  (1) Exclude compulsive symptoms secondary to other psychiatric disorders, such as schizophrenia, depression, or phobias.
  (2) Exclude secondary obsessive-compulsive symptoms of organic brain diseases, especially basal ganglia lesions.
