Paste moxibustion for cold rheumatic joint pain

  To observe the clinical efficacy of plaster moxibustion method and to provide a satisfactory special therapy for the treatment of wind guan pain. Methods: 147 cases of wind guan pain were treated with traditional moxibustion method, and the clinical efficacy was observed. Results: 128 cases were cured recently, accounting for 87.1%, 12 cases were effective, accounting for 8.2%, and 7 cases were effective, accounting for 4.8%, with an overall efficiency of 100%. Conclusion: In this group of cases, the effect of plaster moxibustion method in the treatment of Fengguan pain is very satisfactory.
  Wind-damp cold arthralgia (referred to as wind guan pain) is a disease caused by the body feeling wind-cold-damp evil to muscle and joint pain as the main manifestation. The author has been trying to treat this disease with traditional moxibustion method since 1997 and has achieved satisfactory results, which are reported below.
  1, clinical data
  1.1 General information
  There were 147 cases in this group, 61 men and 86 women; the minimum age was 28 years old, the maximum age was 72 years old, and the average age was 47.6 years old; there were 58 cases of knee pain, 21 cases of back and lumbar pain, 12 cases of elbow pain, and 56 cases of knee joint combined with other joints and parts of pain.
  1. 2 Diagnostic basis
  All cases in this group were diagnosed in strict accordance with the criteria of the 1985 National Academic Symposium on Combined Chinese and Western Medicine and Rheumatic Cold Diseases in some provinces and cities (formulated in 1985 and revised in 1988 at the National Academic Conference on Combined Chinese and Western Medicine and Rheumatic Diseases in Kunming).
  (I) Medical history There is a history of attack by rheumatic cold evil.
  (II) Symptoms Some joints or muscles are sore, numb, painful or even excruciatingly painful, with difficulty in moving; the condition is aggravated by cold or weather changes (cloudy, rainy, windy).
  (III) Signs The affected joints are limited in function due to pain, but are relieved after activity. Most cases are painful but not swollen, and a few cases have mild swelling around the joints (no redness or heat).
  (iv) Laboratory tests: ESR is normal in most cases, but slightly faster in a few cases; ASO, RF, blood count, etc. are all normal.
  (E) X-ray examination Except for a few cases where soft tissue swelling is seen, there is generally no bone change. Due to the long-term stimulation of wind-damp and cold evil (especially wet or cold-damp evil), some cases may be complicated by osteophytes, so X-ray should be taken to exclude.
  (F) Prognosis After remission or cure, no deformity is left in the affected joints, and joint function is restored to normal.
  2.Treatment method
  2.1 Select acupuncture points
  1) Take the anointing point method: the patient sits flat on the bed, bend the knees against the chest, cross the forearms, hold the hands on the knees, lower the head, the forehead against the back of the hands, make the two scapulae fully open, under the spine of the fourth thoracic vertebrae, the medial edge of the scapulae bone seam pressure, feel the pain between the chest ribs, spread to the arm, that is the anointing point, pinch marks to mark.
  2) Matching points: qihai and sansili; for cold pain in the back of the foot, add zhiyin point.
  2.2 Operation
  Firstly, moxibustion on the Anointing Point with large moxa cones, 13 strokes each time; then make the patient lie down, take the three points of Qihai and Foot Sanli, moxibustion with large moxa cones, 7 strokes each. If you need to add moxibustion for Zhiyin point, you can do it at the same time as moxibustion for Ancient Compass Point, with seven strokes of small moxa cones on each side.
  2,3 Treatment course
  Once a day, 15 days as a course of treatment, with a three-day break between courses.
  3. Observation of therapeutic effect
  3.1 Therapeutic efficacy standard
  According to the revised standard of the 1988 Kunming National Symposium on Combined Traditional Chinese and Western Medicine and Rheumatic Diseases. Recent cure: the pain of the involved joints and muscles disappears after treatment. The joint function is normalized, and the ESR is reduced to normal for those who are fast; effective: the pain of the involved joints and muscles is mostly disappeared or significantly reduced; the ESR is normalized, but the joint pain is not disappeared; effective: the pain or swelling of the involved joints is improved; invalid: the symptoms of the involved joints are not improved after more than 1~2 courses of continuous treatment (15 days is 1 course of treatment).
  3.2 Treatment results
  Recently, 128 cases were cured, accounting for 87.1%; 12 cases were effective, accounting for 8.2%; 7 cases were effective, accounting for 4.8%. The total effective rate was 100%. The shortest treatment time was three times, and the longest was 2 courses of treatment for another 7 days.
  4.Typical cases
  Yao Moumou, female, 43 years old, teacher. Consulted on January 19, 2000. Complaint: cold pain in the knee joint for more than three years, recently aggravated, accompanied by a cold back, cold below the ankle and back of the foot, just when the weather changes, the back of the knee, shin and foot is not warm all night. He had previously taken ibuprofen and other drugs, as well as acupuncture and physical therapy, but the effect was not significant. Examination: no local erythema, ESR, ASO and RF were normal. The diagnosis was rheumatic cold arthralgia. He was treated with moxibustion method once, and the cold symptoms in the back were significantly reduced, and the cold pain in all parts had completely disappeared after three times of treatment. 2 years of follow up without recurrence.
  5. Discussion
  5.1 About rheumatic cold arthralgia
  Rheumatic cold arthralgia is a new disease name proposed in recent years. The cause of this disease is still unknown in modern medicine, and there is no special discussion on it. In the past, it was mostly called rheumatic pain and benign arthritis in clinical practice. Recent studies have found that the clinical manifestations of this disease are mostly painful, and there is no local inflammatory manifestation of redness, swelling and pain in the affected joints. The disease is characterized by aggravation in cold and weather changes (windy, cloudy, rainy). Based on the pathogenesis and clinical features of this disease, it is believed that rheumatic cold is the pathogenic factor of this disease, and it is an independent disease, which was named rheumatic cold arthralgia in 1974.
  Its pathological changes can be broadly divided into three types: inflammatory, proliferative, and mixed, and the clinical diagnosis of Chinese medicine classifies it into five types of evidence: wind-heavy, wet-heavy, cold-heavy, rheumatic, and cold-damp. In this group, the moxibustion method is the general treatment method of “all winds and solid paralysis”, so no classification was given. The prognosis of the disease is good, and there are no sequelae after the cure. At present, in addition to Chinese and Western medicines, various kinds of physical therapy are used, but the treatment with traditional moxibustion method and satisfactory results have not been reported.
  5.2 About the Anointing and Moxibustion Method
  It was firstly published in the “Thousand Gold Essential Formula”, Volume 37, and later collected as “Anointing and Moxibustion Method”, Volume 2. The unique point of this method is that first of all, emphasize the body position posture of the point, must make the two scapulae fully separated, “tendons and bones empty, press the patient feel traction in the chest and ribs, finger pain, that is, the real point.” Second, the number of moxibustion should be more, “moxibustion to a hundred strong thousand strong” . In the modern clinical situation, it is generally appropriate to moxibustion more than ten strong, this group of cases moxibustion thirteen strong. Thirdly, after moxibustion, we must moxibustion Qihai and Foot Sanli three points, “to lead the fire to the bottom”, to prevent the congestion of fire on the top. The point is not found in the “Mingtang”, “A B” two scriptures, later people will be listed in the bladder meridian.
  In the past, the Qin slow not save the disease of the Marquis of Jin, is the disease into the vitals of the reason. Sun Simiao said: “when people clumsy, can not seek this point, so the persistent disease is difficult to condemn, if you can be attentive to the convenience of seeking moxibustion, no disease is not cured.” The acupuncture and moxibustion into the cloud: “(moxibustion point) main treatment of Yang Qi deficiency, the wind solid cold ……” the author used to treat wind pain, through moxibustion point, and with the sea of Qi and other points, stimulate the meridian, warm through the meridians, fire to dispel cold, dispersing wind and dampness, support for the evil, the symptoms of the two Gu It can help to promote the root of the problem. It is a shortcut for rheumatic cold joint pain.
  Although the operation of moxibustion is a little tedious, and there is the inconvenience of moxa smoke, but it is still a good method for this kind of stubborn disease which still lacks special treatment.
