How should diabetic foot ulcers be treated?

  There are many patients who consult about how to treat diabetic foot ulcers, so I will give a simple answer first: diabetic foot ulcers are generally divided into two types, the first is the ordinary diabetic foot ulcers without combined vascular blockage, this kind of ulcers through timely professional drug exchange treatment, the external use of some special Chinese medicine, with the nibble clearing techniques, as well as oral medication and infusion, generally heal quickly, but healing After the healing, you have to continue to take some herbs to prevent recurrence.  The second type of diabetic foot ulcer is combined with blocked blood vessels, this kind of ulcer does not heal because the blood supply is not enough, if the blood supply of blood vessels is not improved, even with the best topical drugs, the wound will not grow, then we must first find a way to let fresh blood transport to the foot, about half of the patients can slowly improve the blood supply through conservative methods such as Chinese medicine and stem cell transplantation, and then with the aforementioned wound-clearing techniques and In the other half of the patients, due to severe blockage of blood vessels, conservative methods are too late to open the blood vessels, so minimally invasive surgery is needed to open the blood vessels directly in order to heal the ulcer and preserve the limb. The treatment period for such diabetic foot ulcers is longer and more expensive, whether it is medication or surgery.  Therefore, if a diabetic patient has an ulcer on the foot, it is imperative that he or she come to the vascular department to check the vascular condition and see which condition it belongs to, whether it is a blocked vessel or a smooth vessel. If you are a foreign patient and do not have the conditions to do this kind of professional vascular examination, you should go to do an ultrasound of the arteries of the lower extremities as soon as possible to see the vascular condition and decide the treatment plan after it is clear. Treatment without understanding the vascular condition is blind treatment, which makes patients spend more money or a small thing, but the key is to delay the disease.  Up to today, I have been engaged in the treatment of diabetic foot for exactly 16 years, and my experience in the diagnosis and treatment of diabetic foot has been obtained only by learning and accumulating little by little. At the beginning, I used the viewpoint of general surgery to look at the infection of diabetic foot, and carried out extensive debridement and toe amputation, but the effect was not satisfactory.  Later, when I saw the introduction of diabetic foot by old professor Li Shiming, I knew that diabetic foot is actually caused by many factors such as neuroendocrine, so I turned to the basic treatment of diabetic medicine, and the effect improved a bit, but I still could not receive full effect for ulcer. However, it was still not fully effective for ulcers, because many diabetic foot problems cannot be treated by internal medicine alone.  Later on, as we saw that the external medicine we used was very effective, such as the red powder ointment handed down by Mr. Zhao Bingnan, Hui Yang Sang Muscle Ointment, Nail Twist, etc., which could heal many difficult wounds, we followed the footsteps of Mr. Lu Peiwen for a period of time afterwards, mainly devoted to the research and application of external Chinese medicine preparations, and explored and explored the means to dispel decay and regenerate muscle, Hui Yang Sang Muscle, and activate blood to regenerate muscle, etc., and I should say The efficacy of diabetic foot has been further improved, but many diabetic foot patients still cannot successfully preserve their limbs because there are still few ways to deal with vascular blockage. At this point, I saw that the treatment of diabetic foot could not be considered complete without revascularization.  However, due to the special nature of diabetic vasculopathy, the success rate of traditional bypass surgery was not high, especially for small vessels below the knee, and many patients had blockages again soon afterwards. Fortunately, we have caught up with the good times, since 2005, the interventional technology for small sub-knee vascular lesions, which was first carried out by some doctors in the United States, Germany and Italy, has been introduced to China, and I have also carried out this technology earlier by learning from Prof. Xiaoming Zhang and Prof. Baixi Zhuang, and have successfully opened blood vessels for many diabetic patients and preserved their limbs.  Although minimally invasive interventional surgery is different from traditional bypass surgery in that it can be done repeatedly, it still brings pain and economic burden to patients. how to make the wound of the foot heal as soon as possible.  Regarding the promotion of wound healing, I am still relying mainly on the above-mentioned special topical herbal preparations and patient nibbling to clear the wound, the effect of some is not bad, but some is still not good, the wound healing is very slow, we reflect on our own thinking, is the diabetic foot treatment has encountered a bottleneck again, can not be overcome?  In 2009, Mr. Xi Jiuyi from Shanghai reminded me of the viewpoint of the Vulcan school: our understanding of diabetes is always stuck on the viewpoint of “Yin deficiency and fire”, and many patients are already very weak, but we still dare not use a lot of tonic drugs for fear of aggravating the deficiency fire. In fact, many patients are “yang deficiency”, after yang deficiency, the limbs are cold, the wound can’t get the warmth of yang, so it naturally can’t grow. In my subsequent treatment, I gradually tried to use warming Yang medicines such as Radix Aconiti, and I really received good results.  Regarding the problem of restenosis after the opening of blood vessels, in addition to using anti-platelet drugs such as aspirin and Bolivar, which are commonly used by western medicine colleagues, I also added my own “Four Flavors Tongue Tang” herbal medicine, which is more effective than western medicine alone. Diabetic foot is a very complicated and multidisciplinary disease that is difficult to treat. Although I am able to use Chinese medicine, Western medicine, drugs, surgery, internal treatment and external treatment to treat this disease, and I can choose a more reasonable treatment plan for patients, I still dare not say that I am the so-called “authority” on this disease. There is no authority on this disease, and the power of individuals is ultimately limited.
