Heavy menstrual flow can be seen in dysfunctional uterine bleeding, mostly due to dysfunction of the hypothalamic-pituitary-ovarian axis, which can be divided into two categories: ovulatory and anovulatory. Patients with large uterine fibroids or adenomyosis have heavy menstrual flow and the presence of blood clots, and patients are often accompanied by epigastric pain and dysmenorrhea. In adolescent girls, because the ovarian function is not fully developed, most of the menstruation at this time belongs to anovulatory menstruation. Anovulatory menstruation has no corpus luteum, resulting in a lack of progesterone, so the endometrium is only in the proliferative phase and does not reach the secretory phase, resulting in incomplete shedding of the endometrium, which affects the contraction of the uterus and can lead to excessive menstrual bleeding. Heavy menstrual bleeding and clots belong to the category of blood stasis, and two causes exist, including qi stagnation and blood stasis and cold clotting. Qi stagnation and blood stasis can be treated with acupuncture and Chinese medicine in combination with herbs to soothe the liver, regulate qi and relieve depression. Cold blood stasis can be treated with acupuncture and moxibustion to warm the meridians and activate the channels, which has a good therapeutic effect.
