Ms. Wang, an elderly patient, recently experienced irregular vaginal bleeding after four years of menopause and came to the Department of Obstetrics and Gynecology at Peking University First Hospital. The pathological results confirmed that she unfortunately had endometrial cancer. More unfortunately, the type of cancer she had was a highly malignant mixed plasma papillary carcinoma! It is highly susceptible to spread and early distant metastasis, and the nuclear magnetic examination performed by the doctor also indicated that the patient had enlarged lymph nodes in the iliac vessels – surgery was urgently needed and time was running out! However, a huge problem appeared in front of the doctors who were going to operate on her: Ms. Wang’s fragile heart function could not withstand the cancer surgery! She had suffered from rheumatic heart disease for more than 20 years, and the examination revealed that her heart had severe joint valve disease, with echocardiography indicating severe stenosis and incomplete closure of the mitral and tricuspid valves, severe incomplete closure of the aortic valve, significant enlargement of the left heart and right atrium, and pulmonary hypertension. Once heart failure occurs, medical treatment will be ineffective and the patient will face the risk of sudden death, not to mention surgery! However, how can obstetrician and gynecologic oncologists stand by and watch the malignant tumor eat away the patient’s body? Even if there is a glimmer of hope, we can’t give up the chance to extinguish the tumor and prolong the life of the patient! Director Liao Qinping of Obstetrics and Gynecology requested the medical office to coordinate a hospital-wide consultation, and the chief physicians of all sister departments such as cardiac surgery were all present to fully evaluate Ms. Wang’s condition and discuss the treatment plan together. In the end, a joint surgery was decided: heart surgery for valve replacement and OB/GYN surgery for endometrial cancer were performed at the same time! This surgery was an unprecedented challenge for the doctors, and the risks and difficulties could be imagined. On March 27, after a series of thorough preoperative preparations, the operation was carried out as scheduled. Director Wang Dongxin of the Department of Anesthesiology led the team to provide smooth anesthesia for the patient and ensure the safety of the operation. Deputy Chief of Cardiac Surgery Dr. Xihui Li with his team performed mitral valve replacement and aortic valve replacement and tricuspid valvuloplasty. After 7 hours passed, the intense surgery was successfully completed and the patient’s hemodynamics were well corrected. Finally, Director Liao Qinping of the Department of Obstetrics and Gynecology with his team boarded the operating table and started to perform endometrial cancer tumor cell reduction for the patient. After another two hours passed, the patient’s uterus, bilateral adnexa and enlarged lymph nodes were successfully removed and the surgery was completed successfully as scheduled, and the patient’s vital signs were stable during the surgery! At this point, the two major surgeries were performed at the same time and ended successfully as planned! This OB/GYN malignant tumor patient with severe cardiac comorbidities was finally saved! The unity of PKUH people, through joint efforts, has once again created a medical miracle! Not only did they perform a highly difficult and complex joint surgery for the patient, but they also did their best to reduce the financial burden for this unfortunate patient. It is unfortunate to have a malignant tumor, but how fortunate is it to receive such careful treatment in this hospital?
