The difference between the five tests for hepatitis B, major triplet and minor triplet, is which is positive for e antigen and e antibody. If hepatitis B surface antigen is positive, e antigen is positive, and core antibody is positive, it is a major triplet, and if hepatitis B surface antigen is positive, e antigen is negative, e antibody is positive, and core antibody is positive, it is a minor triplet. Major triple positive indicates active hepatitis B virus replication, high hepatitis B virus DNA level and high infectiousness, while minor triple positive indicates reduced hepatitis B virus replication, low or negative hepatitis B virus DNA level and low infectiousness. Whether it is major triplet or minor triplet, it is necessary to regularly observe whether the transaminases are continuously or repeatedly elevated, and to take into account the patient’s age, whether there is a family history of cirrhosis or liver cancer, etc. If necessary, liver histological examination by hepatic aspiration will be done to decide whether antiviral treatment should be given.
