Laparoscopic horseshoe hydronephrosis V-Yplasty

  Laparoscopic reconstructive surgery for hydronephrosis of the horseshoe is a very technically demanding type of ureteral reconstruction surgery. The transabdominal route is the best choice. This case of transabdominal laparoscopic surgery for hydronephrosis of the horseshoe is a demonstration case in my advanced medical lectures in urology at Peking University, which is summarized to share the experience.  Patient, male, 24 years old, main cause: left lumbar pain for 1 year, horseshoe kidney with bilateral hydronephrosis found for 40 days was admitted to the hospital.  Diuretic renal dynamic examination: GFR: left 17 ml/min right 59 ml/min. Preoperative CT: horseshoe kidney with severe hydronephrosis of the left kidney.  Retrograde angiography: left pelvic ureteral junction obstruction, severe hydronephrosis of the left kidney.  Surgical method: transabdominal laparoscopic V-Y pelvicoplasty of the left kidney, with an operative time of 1.5 hours and bleeding of about 20 ml. The picture shows the spacious funnel shape of the pelvic ureteral junction after the pelvicoplasty.  The surgical result was good: the D-J tube was removed one month after the operation, and the left back pain disappeared and the left hydronephrosis disappeared at the three-month review.  Postoperative CT: the left hydronephrosis disappeared.
