What is premature ejaculation? The rapid ejaculation that occurs during the first sexual intercourse, stressful work or physical fatigue is usually temporary or episodic. The reason is that most men who try to have sex for the first time have little sexual experience, and the various aspects of sexual life are not fully coordinated and less skilled. In addition, the effects of extramarital affairs, poor living privacy, fear of pregnancy, nervousness, and female factors can also lead to the occurrence of rapid ejaculation. Therefore, rapid ejaculation during sex may also be caused by occasional psychological and physiological states, not always true premature ejaculation.  3. Are premature ejaculation and impotence the same thing?  Premature ejaculation and impotence are both one of the most common sexual dysfunction diseases in men and are often confused together. In fact, they are two different diseases, and there is a big difference between them in terms of onset characteristics, clinical symptoms and treatment. Impotence is medically known as “penile erectile dysfunction” and refers to the inability of a man to achieve and maintain a full erection during sexual intercourse in order to have a satisfactory sex life. Premature ejaculation is a disease in which a man’s ability to control ejaculation is reduced to a certain level and he is unable to control and ejaculate prematurely during sex, which makes his partner fail to achieve sexual satisfaction and affects his relationship with his partner. To put it simply, premature ejaculation is a condition in which a person can have an erection, can penetrate, and can ejaculate, so the solution is to delay the arrival of ejaculation, while impotence is a condition in which a person cannot have an erection or an erection that is not hard enough, or an erection that cannot be maintained, so the solution is the problem of erectile hardness and the maintenance of an effective erection for a long time. Of course, some patients can also have both of these conditions.  4, masturbation is bound to lead to premature ejaculation?  Masturbation is not the cause of premature ejaculation, but it may be associated with it. For men with a history of masturbation, when they masturbate, they are often worried about being discovered by others or are bent on chasing the ejaculatory pleasure and tend to develop the habit of ejaculating in a hurry, which may bring this bad habit to the real act of sexual intercourse and therefore show symptoms of premature ejaculation. However, on the other hand, masturbation may be helpful in terms of ejaculation control for premature ejaculators, and training to control the frequency, rhythm and timing of masturbation may be beneficial in delaying ejaculation.  5. Is dorsal penile neurectomy the best solution for premature ejaculation?  There are many treatment options for premature ejaculation, such as psychological and behavioral therapy, oral medication (SSRIs, traditional Chinese medicine, etc.), local therapy (local anesthetics) and surgery, all of which aim to improve the sensitivity threshold of the penis and adjust the ejaculatory reflex so that the patient can control ejaculation. Some hospitals on the Internet promote “dorsal penile nerve block” as the gold standard for treating premature ejaculation. This argument attributes all causes of premature ejaculation to excessive sensitivity of the dorsal penile nerve. In fact, the ejaculatory reflex is a joint central-peripheral nerve process, and foreign studies now believe that the central nerve plays a more important role in the entire reflex process. Therefore, the root cause of premature ejaculation may not always be in the dorsal penile nerve, even if the dorsal penile nerve is cut, there must be strict indications, and the argument that this surgery is suitable for all patients with premature ejaculation obviously does not stand the test. In clinical practice, the therapeutic effect of dorsal penile neurectomy is much less than expected, and postoperative complications may occur, such as loss of penile sensation, numbness, or even erectile dysfunction. Therefore, both doctors and patients must be cautious about this procedure that is not yet recommended in the guidelines.
