How is carotid artery stenosis treated?

  In recent years, with social development, economic progress and improvement of people’s living standards, changes in dietary structure, changes in working methods and other factors, carotid stenosis has now become a common vascular disease in the middle-aged and elderly, and the age of the disease is decreasing year by year, with a younger development, and the number of young and middle-aged people suffering from carotid stenosis is increasing year by year.  So how is carotid stenosis treated? What is the current situation of carotid artery stenosis treatment in China?  First of all, we will explain the classification of carotid stenosis, which can be divided into five stages, as follows: When the degree of carotid stenosis is 1-49%, it is mild stenosis, which does not have a great impact on blood flow and may not show symptoms, but it should be taken seriously and treated with anticoagulant drugs in advance, and a review of carotid ultrasound should be performed every six months. If the stenosis is between 50% and 69%, the stenosis is moderate, and if the stenosis is between 70% and 99%, the stenosis is severe. In the case of moderate to severe carotid artery stenosis, the stenosis is more serious, the carotid lumen is thinner and the blood flow is less, so surgical treatment should be performed regardless of the symptoms.  Surgical treatment for moderate to severe carotid stenosis is usually done by carotid stenting and carotid endarterectomy. Each procedure has its own indications, and depending on the patient’s specific situation, either carotid stenting or carotid endarterectomy can be chosen. For example, in some patients, the vascular alignment is complex, very tortuous, difficult to reach with a catheter, and the degree of stenosis is too severe; these patients are not suitable for stenting and should undergo endarterectomy; in other patients, due to the inability of their organs to tolerate major surgery and general anesthesia, they should undergo stenting, so it depends on the patient’s own situation.  There is a more serious case of carotid stenosis, which is 100% stenosis, and that is complete occlusion. In this case, the only treatment is usually vascular bypass.
