Trigeminal neuralgia is the most common and most severe pain in neuralgia, and it is not easy to cure. It belongs to the category of headache and migraine in Chinese medicine. There are two types of headache: primary and secondary, and it is most common in middle-aged and elderly women over 40 years old. Modern medicine mostly uses phenytoin sodium, carbamazepine and other drugs or trigeminal nerve block therapy to relieve pain or reduce the number of attacks. Many patients have developed dependence.
If drug treatment is not effective, surgical treatment is used to cut off the trigeminal nerve branches. Since 1993, the author has treated 76 cases of this disease with the acupuncture medical therapy invented by Professor Zhu Hanzhang, and achieved the miraculous effect of simple, fast, inexpensive, safe, reliable and non-toxic side effects. Now we report as follows:
1.Clinical data
Among the 76 cases, 24 were male and 52 were female; the minimum age was 35 years old, the maximum age was 78 years old, the average age was 52 years old, and the duration of the disease was 3-10 years; there were 62 primary cases and 14 secondary cases; there were 9 cases of ophthalmic branch, 28 cases of maxillary branch, 36 cases of mandibular branch, and 72 cases of trigeminal nerve ganglion.
2. Clinical manifestations and signs
2.1 Pain site: limited to the trigeminal nerve innervation area, mostly on one side, with the 2nd and 3rd branches being the most common, while the first branch was rarely involved. Therefore, the pain is more obvious and most intense at the points of the cheek, upper and lower jaws, such as the Sun, Shangguan, Xiaguan and Buccal car. It is not difficult to diagnose this disease according to the anatomical parts.
2.2 Nature and characteristics of pain: The onset of pain is rapid, with typical paroxysmal sharp pain, stabbing pain or cutting or burning pain, and each attack lasts for several seconds or 1-10 minutes.
2.3 Trigger area or trigger area: A certain area in the trigeminal innervation area is particularly sensitive and can be triggered by a slight touch, these areas are called trigger points. Even worse, cold wind, sound and light stimulation or body movement can trigger the head, seriously affecting the patient’s quality of life, even in the interval, the patient does not feel to eat or speak loudly, wash the face, lest trigger.
2.4 The disease generally has a long duration, with a remission period without seizures, usually occurring after several weeks or months. Once the onset of the disease, many consecutive seizures are quite frequent, and the interictal period is gradually shortened, and in severe cases, there can be several or dozens of seizures in a day.
2.5 Neurological examination is often unremarkable.
2.6 Exclude other diseases causing severe facial pain, such as dental pain, paranasal sinusitis, migraine, vascular tension headache, glossopharyngeal neuralgia, mandibular arthritis and trigeminal hemimelia neuroma.
3.Treatment method
Check the pain point that is the needle knife treatment point, purple potion fixed point, local routine disinfection lay sterile towel, wear sterilized gloves, the left thumb fixed pain point, the right hand holds the needle knife, the knife line is generally consistent with the local muscle and nerve direction, then suddenly force into the needle knife, wait for the local soreness and soreness and swelling can be performed when the incision and release 3-5 knife or longitudinal sparing stripping several times can be out of the needle, after the operation, compression The needle hole should be compressed for a few moments after surgery and fixed with a band-aid. For those with heavy clinical symptoms or more frequent attacks, 2% lidocaine 2ml, vitB12 500ug, dexamethasone 5mg can be used for local closed treatment after surgery.
4.Treatment results
In this group of cases, after one treatment, the local tingling and throbbing symptoms were obviously reduced immediately or there was only a local numbness and soreness, but it was not the numbness and electric sensation of nerve injury. 56 cases were cured at one time, and 18 cases had another attack after 24 hours, but they were not cured by the original pain treatment point but by this method. 1 case, after three treatments, the throbbing pain in the trigeminal ganglion and the buccal region of the mandibular branch disappeared, and there was only dull pain at the right lower second molar. In one case, after three treatments, the symptoms of trigeminal ganglion and the throbbing pain in the buccal region of the mandibular branch disappeared, but there was only a dull pain at the right lower second molar, which was secondary.
In one case, although the pain symptoms were relieved after treatment, the pain could not be stopped and recurred and was not cured. The remaining 75 cases were all cured and no recurrence was observed during the follow-up period.
5, typical cases
Case 1 Li××, female, 78 years old, farmer in Renhe Township, Xiping County, suffered from trigeminal neuralgia for more than 5 years, which could be triggered by eating and talking, and had more than 10 attacks per day;
After the treatment, he only felt local numbness and soreness, and after 48 hours, he reported that the number of episodes per day was significantly reduced to 3, and the pain level of each episode was significantly less than before, After three treatments, the trigeminal neuralgia symptoms disappeared, leaving only residual pain at the right lower second molar, which was persuaded to be extracted, and then cured with Chinese and Western medicine.
Case 2 Wang××, female, 65 years old, a citizen of Sanmenxia, had suffered from trigeminal neuralgia for more than 10 years and had been treated with multiple treatments with no effect. The patient immediately felt that the pain in the head and face disappeared and her mind was clear. The patient was asked to return to the clinic for further treatment, but did not do so because of the long distance, and then treated with other methods without success.
