It is possible for otitis media to be painful, and the main clinical manifestations are as follows: i. Acute secretory otitis media, due to the presence of negative pressure in the tympanic chamber and fluid accumulation in the tympanic chamber, patients can see the relevant manifestations such as tympanic membrane congestion and tympanic membrane invagination when they are examined, and the early stage is mostly accompanied by obvious painful sensation in the ear, which is more common especially in children. In acute suppurative otitis media, the tympanic membrane is not perforated. Due to the large amount of purulent secretions stored in the tympanic chamber, the tympanic membrane is congested due to the extrusion of the tympanic membrane, and the pressure inside the tympanic chamber is high, so the patient has obvious pain during the period when the tympanic membrane is not perforated, and the pain will disappear once the pus flows out after the tympanic membrane is perforated. Third, chronic suppurative otitis media, due to the inflammation within the tympanic chamber stimulates inflammation, invades the mucosa within the tympanic chamber as well as the nerves within the tympanic chamber, which can lead to chronic painful sensation in the patient’s ear, and otitis media may also appear as ear pain.
