What should I do about macular hemorrhage in high myopia?

  In recent times, as the weather becomes colder, the rate of vision loss and loss of high myopia patients is increasing in my clinic. Due to the lack of knowledge of macular hemorrhage disease, patients lose the best treatment time, resulting in blindness.  So what is macular hemorrhage?  Due to high myopia, the eye becomes longer and larger, the wall of the eye becomes thinner, the choroid also becomes thinner, the original choroid has fewer blood vessels, the supply of blood nutrients to the retina decreases, the retina is ischemic, lack of nutrients, it will secrete a kind of “vascular growth factor”, vascular growth factor to promote the growth of new blood vessels. Since there is no retinal blood vessel in the macula of the retina, its nutrient supply is completely dependent on the choroid, and the macula is the most sensitive place to see things, which consumes the most nutrients and is most likely to cause ischemia, so the new blood vessels are most likely to grow under the retina in the macula. The growth of neovascularization is a good thing and a kind of compensation, but because the wall of neovascularization is very thin compared with normal blood vessels, only – layer of cells, it is very easy to rupture and bleed, which is the macular hemorrhage area seen in the clinic, if there is not much macular bleeding, it can be absorbed in 2-3 months and most of them do not leave sequelae, if the amount is more, it can form a paralytic scar, forming a Fuchs’ spot and leaving a permanent dark spot in the center of the visual field. Since the sub-macular neovascularization still exists, the root cause of bleeding still exists, and the neovascularization keeps rupture, forming repeated bleeding and repeated bleeding is very easy to form scars. It leads to permanent vision loss.  What should I do about macular hemorrhage?  In the past years, when encountering such patients, only symptomatic treatment was available, but it was often very heartbreaking to see most patients go blind.
