Inflammatory bowel disease patients treating hemorrhoids, beware of adding insult to injury

People may be unfamiliar with inflammatory bowel disease because it is indeed a relatively rare disease in our country, so it is normal that it is under-recognized. What is inflammatory bowel disease? The so-called inflammatory bowel disease is a non-specific chronic inflammatory bowel disease, mainly including ulcerative colitis and Crohn’s disease, the former mainly damages the colon and rectum, and the latter can damage any gastrointestinal area from the oral cavity to the anus, and it is more common in the end of the small intestine and colon. If a patient has hemorrhoids on top of inflammatory bowel disease, it is not an exaggeration to say that it is “worse than worse”. This is not only because of the existence of independent pathological basis of such patients, more importantly, the difficulty of treatment of such patients is also extraordinary. 1, inflammatory bowel disease symptomatic hemorrhoids independent of the pathological basis Inflammatory bowel disease is a non-specific chronic inflammatory bowel disease, but inflammatory bowel disease patients with symptomatic hemorrhoids, may be independent of the pathological basis of intestinal inflammation, hemorrhoids are not inflammatory patients with a unique performance, mainly due to the presence of chronic diarrhea caused by patients. With the postponement of age, connective tissue degradation, relaxation, fragile, coupled with a variety of connective tissue fiber damage, degeneration and a variety of factors that make the anal cushion by the impact of the downward movement of the role of factors, and the loss of its role in fixing the anal cushion downward, the blood vessels tortuous, dilation and thickening of the anal cushion into hemorrhoids. At this time the hemorrhoids are mostly accompanied by symptoms, so it is called symptomatic hemorrhoids, before the need for appropriate treatment. Inflammatory bowel disease accompanied by hemorrhoids patients treatment is difficult up is not an easy task, compared to patients who simply suffer from hemorrhoids, the difficulty and some of the corresponding risk, is considered to be “super double”, especially inflammatory bowel disease is still in the active phase of the patient, the choice of treatment needs to be careful and prudent. 2, the choice of treatment Inflammatory bowel disease in the active phase of hemorrhoid surgery is more dangerous, the trauma caused by the surgery may lead to complications more difficult to deal with than hemorrhoids. The limited research data available suggests that the risk of serious complications after hemorrhoidectomy in patients with inflammatory bowel disease is significantly higher than in patients with non-inflammatory bowel disease. This may lead to spreading inflammation, persistent bleeding from the wound, and even a risk of developing blood clots. The impact of postoperative risks is even more dramatic when compared to the symptoms caused by hemorrhoids, such as discomfort associated with localized anal bleeding and pain. Therefore, during the acute or active phase of inflammatory bowel disease, conservative treatment is preferred, including improvement of lifestyle habits, dietary modifications, and medication. Improvement of living habits, such as moderate exercise, smoking cessation and alcohol restriction, regular work and rest, avoiding late nights and overwork. Dietary adjustments, on the other hand, should emphasize the long-term destructive effects of western dietary structures such as cheese and cream on the intestinal tract, and avoid highly allergic foods such as seafood, shrimp, crab, barbecue, hot pot and other foods. More intake of high fiber foods, such as green leafy vegetables, coarse grains, fruits and so on. When the intestinal disease is in remission, and conservative treatment has not been effective, patients can be elective hemorrhoidectomy. Hemorrhoids patients undergo surgical treatment although can achieve the purpose of radical cure, but after the radical cure there is still a risk of recurrence, the above preventive measures for the prevention of hemorrhoids recurrence there is also a certain significance, can be said to be “two birds with one stone”. So these suggestions should not be taken as a “wind in your ears”, good health habits need to be implemented every day. References [1]. China hemorrhoid diagnosis and treatment guidelines (2020)[J].
