The overall treatment of femoral head necrosis is classified into two categories: joint replacement and head preservation therapy. Joint replacement is the last step and should be avoided and postponed as much as possible. There are many methods of head preservation treatment, and a simple and effective option should be chosen for this purpose. Overall, comprehensive treatment is required, but also individualized treatment, based on disease stage, causative factors, personal requirements, marital career needs, and self-perception. By comprehensive treatment, we do not emphasize one aspect, such as stem cell transplantation, such as medullary core decompression, such as tantalum rod implantation, etc. According to the pathophysiological process of osteonecrosis, medullary core decompression should be necessary to improve local blood flow, and on this basis, stem cell or red bone marrow transplantation should be beneficial, but in order to further improve blood flow, anticoagulation, vasodilation, volume expansion and blood activation should also be performed, of which Chinese medicine is a very important means. Of course, tantalum rods or autologous/allogeneic bone grafts, including fibula grafts, are necessary for mid-stage patients, but they are not necessarily necessary for relatively early stage patients. In addition, head preservation treatment should be performed as early as possible, and the earlier the treatment, the better the results.
