The patient, a 69-year-old female, was admitted to the hospital with “abdominal distension and bilateral lower limb edema for more than 20 days”. She had been taking “earth panax quinquefolium” for more than 10 days before January due to trauma. She had no history of viral hepatitis, no history of schistosomiasis, and no history of alcoholism. Physical examination: T: 36.9℃ BP: 130/70mmHg. The mental state was clear and slightly atrophied. No liver palm and spider nevus were seen, and the skin sclera was mildly yellowish. Jugular venous filling was not obvious, respiratory sounds of both lungs were clear, no obvious dry and wet rales were heard, heart rate was 80 beats/min, heart rhythm was uniform, no pathological murmur was heard in the auscultation area of each valve, abdominal distension, no varices in the abdominal wall, no pressure pain and rebound pain in the whole abdomen, liver was 50px below the right rib, soft, spleen was not touched under the rib, no percussion pain in the liver area, positive mobile turbid sounds, mild edema in both lower limbs. Laboratory tests: blood biochemistry: TBiL:35.1umol/L, DBiL:19umol/L, ALB:36.7g/L, ALT:124U/L, AST:161U/L, GGT:176U/L, ALP:299U/L. Coagulation function: APTT:43.6s, D dimer:714ng/ml. abdominal ultrasound : Large accumulation of fluid in the abdominal cavity. Abdominal CT: large liver, reduced density, map-like” changes, poorly displayed hepatic veins, large amount of ascites. Viral hepatitis markers were negative. He was diagnosed as hepatic small vein occlusion, and was treated with low molecular heparin, salvia and prostaglandin to improve microcirculation, and reduced glutathione to protect the liver and diuretic, etc. He was discharged after 20 days. The treatment was healed after 20 days. Clinical symptoms similar to cirrhosis may appear, including jaundice, ascites, hepatosplenomegaly and abnormal liver function. The disease lacks effective treatment and is all symptomatic with support. Early treatment is more effective, but late mortality is high, with an overall mortality rate of about 50%. The best way is to avoid taking earth pansy.
