Pituitary tumor surgery should choose different ways according to the size of the tumor, where it is located, the direction of growth, and the relationship with the surrounding tissue structures. Generally, there are two types of surgery: transfrontal craniotomy and transnasal butterfly microsurgery.
1. Transnasal butterfly microsurgery.
(1) Unipolar electrocoagulation incision of the nasal septum mucosa up to the nasal septal cartilage, and stripping the nasal mucosa from the nasal septal cartilage.
(2) Place a spacer into the gap between the incised septal mucosa and the septum to expose the bilateral pterygoid sinus openings.
(3) Open the anterior wall of the pterygoid sinus to expose the saddle floor, open the saddle floor bone to expose the dura mater, and then incise and perform resection of the tumor.
2. Transfrontal craniotomy: select the position, incision and bone window, cut the dura mater, enter through the pterygoid point, pass through the surgical gap after lateral fissure dissection, expose the tumor, and perform resection.
