The application of Chinese medicine somatology in the treatment of malignant tumors

Chinese medicine treatment for malignant tumors is an important member of the family of comprehensive tumor treatment, which has been paid more and more attention. In China, nearly 100% of cancer patients are receiving TCM treatment, which is of great concern due to the different forms and methods and inconsistent efficacy. How exactly should TCM treatment be applied to cancer patients? How long should it be applied? It is a question that needs to be urgently explored. In recent years, with the development of TCM somatology, its theory has been widely used in various clinical departments of TCM. Now, we apply this theory to discuss the above problems, and discuss with our colleagues, so as to draw in the jade with its brick. Chinese medicine believes that the human body and the balance between the human body and the nature is a balance of yin and yang, once this balance is broken for various reasons, diseases will arise. Malignant tumors are often caused by the weakness of the body due to the lack of positive energy and the weakness of the body, and then by the influence of external factors such as the invasion of evil toxins, environmental deterioration, long-term dietary disorders, labor and relaxation, and emotional and mental disorders, etc. The recurrence and metastasis of tumor patients after surgery and radiotherapy are also caused by the decline of the body’s constitution and the disruption of the internal balance. Therefore, for Chinese medicine treatment of tumor patients after surgery and radiotherapy, the author believes that the purpose is not to kill the tumor cells themselves, but to adjust the biased constitution of yin and yang, so that the tumor loses the platform for generation, recurrence, metastasis and progress and deterioration, thus achieving the purpose of tumor treatment. At the same time, malignant tumor is a chronic disease, and its treatment has the characteristics of long treatment period and obvious characteristics of treatment stage compared with other diseases. Therefore, the recurrence and metastasis of tumor patients are closely related to their physical condition, and the adjustment of patients’ physical condition is especially important in the treatment of tumor by TCM. Chinese medicine has always taken human body as the research object. In recent years, TCM doctors often emphasize the importance of evidence-based treatment in clinical diagnosis and treatment, and believe that only evidence-based treatment is the core and essence of TCM theory, but often find that the treatment results are not satisfactory, the reason is that there are different physical differences among patients with the same symptoms, and the physical differences have obvious correlation with the occurrence and prognosis of the disease. The same disease of different physique has different symptoms and prognosis, and the treatment is also different; different diseases of similar physique have similar symptoms and prognosis, and the treatment is also similar, which is called “different treatment for the same disease” and “different treatment for the same disease”. Therefore, from the 1970s, Wang Qi and other scholars began to study the doctrine of physique, and in 1978, the concept of “doctrine of physique in Chinese medicine” was formally introduced. The “TCM Physique Theory” is a discipline guided by the theory of TCM to study the physiological and pathological characteristics of various types of human physiological heat, and to analyze the reflective state of diseases, the nature of lesions and the development tendency, so as to know the prevention, treatment and health rehabilitation of diseases. TCM constitution refers to the integrated and relatively stable inherent traits of morphological structure, physiological function and psychological state formed during the life of human body based on innate endowment and acquired, which are the human personality characteristics formed during the process of growth and development that are compatible with the natural and social environment. Wang Qi et al. have identified the morphological structure, physiological function and psychological state of individuals, and through large-scale epidemiological surveys, classified the physical types into nine types, namely pinghe, qi-vacant, yang-vacant, yin-vacant, phlegm-damp, damp-heat, silt-blood, qi-yu and special endowment. Among them, except for the pinghe, the remaining eight types of constitution will have an impact on the occurrence and regression of diseases under specific circumstances. From the point of view of the formation process of constitution, first of all, individual constitution inherits its own family genetic characteristics and is a regular process of continuous evolution in the process of growth and development, so it has the characteristic of stability, and at the same time, individual constitution is influenced by climate, diet, emotion and various social environments, and will make corresponding adaptive changes. The author found that tumor patients can also change to a peaceful constitution through long-term adjustment of Yin deficiency, Qi deficiency, Damp heat, Phlegm dampness, etc. Therefore, the constitution is adjustable, and the use of Chinese medicine to adjust the biased constitution is an important means to prevent the occurrence and deterioration of diseases. 3. Correlation between physical factors and malignant tumors 3.1. Physical constitution and tumor development In the same environment, tumors occur in different people and some do not. The material basis of physique is the level of qi, blood and fluid of the internal organs of human body, and the difference of physique is also due to the different composition and function of qi, blood and fluid of internal organs, and the strength and weakness of human body’s positive qi is also determined by the above basic material. Modern research also shows that the imbalance of oncogenes and oncogenes in human body caused by various external factors is an important cause of tumor development, and the expression of oncogenes varies in different body types. Therefore, physical factors are closely related to tumor development. 3.2. Body constitution and tumor prognosis The level of human body’s positive energy and the severity of sensing evil are two important factors to determine the prognosis of the disease. After surgery and radiotherapy, the tumor has been removed and the patient is in a tumor-free state from the point of view of disease, so the evil qi has been removed and theoretically no treatment is needed, but 60-70% of patients still have recurrence and metastasis. For patients with advanced tumor, the positive energy and the evil energy compete with each other, and the positive energy is gradually deficient and the evil energy is flourishing. Although the deterioration of tumor at this time has a great relationship with the strength and weakness of the evil energy and whether the treatment is appropriate, the positive energy is flourishing to counteract the evil energy, which can slow down the progress of the disease. Therefore, physical factors are closely related to tumor prognosis, and adjusting body can prevent and delay recurrence and metastasis and improve survival quality. 4.Application of somatic theory in tumor treatment At present, the treatment of malignant tumor is mainly divided into two kinds of adjuvant treatment and palliative treatment, and the treatment means are mainly surgery, radiotherapy, chemotherapy, biological treatment and Chinese medicine treatment. Whether it is adjuvant or palliative treatment, TCM is always involved in it. For patients undergoing surgery and adjuvant radiotherapy, they are already in a tumor-free state clinically, and the purpose of clinical treatment is to prevent recurrence and metastasis. At this time, the use of Chinese medicine is mainly to regulate the body constitution, by adjusting the patient’s unbalanced constitution and improving his or her positive energy, so that the tumor loses the basis of recurrence and metastasis to achieve the purpose, and the treatment period is long. The dosage of medicine should be light and the taste of medicine should be small, 15-20 flavors is better, and the application of toxic drugs should be avoided to prevent damage to the body. During the period of body conditioning, if relevant symptoms appear due to the influence of internal and external environment, the prescription can be temporarily changed to mainly treat the symptoms, and the dosage and flavor of the medicine should be increased as needed, so that the symptoms can be corrected within 1-4 weeks to distinguish them from the pathological constitution. At the same time, considering the toxicity of Chinese medicine and the possible toxic side effects caused by long-term use, attention should be paid to the time of taking Chinese medicine. Because 1-3 years after surgery is often the high incidence of tumor recurrence and metastasis, therefore, the author believes that the postoperative application of traditional Chinese medicine should not exceed 3 years in general, and if the body constitution becomes calm during the medication period, consideration can be given to stop taking traditional Chinese medicine. For patients with tumor in late palliative treatment, clinical evidence-based treatment should be the main focus, and on the basis of evidence-based treatment, supplemented with body identification treatment. And appropriately add heat-clearing and detoxifying drugs, such as white hairy vine and three-leaf green for lung cancer patients, vine pear root and tiger root for colorectal cancer, and cat’s claw herb and nail piece for breast cancer. At this time, we should combine the identification of evidence and body with the identification of disease. Because the body constitution consists of several parts, such as morphological structure, physiological function and psychological factors, patients can be advised to improve inappropriate diet and lifestyle based on the use of traditional Chinese medicine and supplemented with food therapy to adjust the body constitution through the bias of food. At the same time, psychological interventions and other methods are used to improve the influence of patients’ bad emotions on their physique. 5. Problems 5.1. The correlation between individual physical factors and tumor onset, recurrence and metastasis is found to exist through some clinical observation studies, but the exact correlation and the degree of correlation cannot be determined yet, and it needs more in-depth clinical studies and epidemiological surveys through proper development of body mass scale to be clear. 5.2. The process of using traditional Chinese medicine to regulate the body is a process of transformation from non-peaceful to peaceful constitution, and clinical observation shows that it can improve the survival quality of tumor patients, but whether it can really prolong the time to disease progression (TTP) and the overall survival of tumor patients is still a question worth studying.
