Fatal “hip” fracture

  This is a patient I personally managed, and I was so impressed.  A 93-year-old woman went out to buy vegetables early in the morning and fell down: her right femoral neck was fractured. When she was sent to the hospital, the hospital said she needed surgery, but her son was so filial that he couldn’t bear to see her suffer so much at such an old age, so he chose to stay in bed for conservative treatment. The old lady was lying down for more than a month. Slowly, the breath began to struggle, and people’s spirit is getting worse. The son hurriedly sent the old man to Changhai Hospital, and the old man received my main consultation group.  In the face of the family’s questions, I summarized down a sentence: open, there is a chance, not open, no chance at all. The assessment came down to the fact that the old lady’s general condition was already very poor, with chronic respiratory failure, decubitus ulcers, and the whole person lying in bed with only her chest constantly panting. We hired the best anesthesiologist in the hospital, and the operation was over in only half an hour with the full assistance of the anesthesiologist. However, the old man’s nightmare had just begun. The incision did not heal but kept oozing due to the respiratory failure that had already occurred before the surgery, the extreme water-electrolyte disturbance, and the severe water-sodium retention. 10 days of medication changes, every day, and as expected, the worst fear happened, the wound became infected.  Infection can be fatal for joint replacement. The doctors wanted to operate again to clear the wound, but the old man’s body was so weak that it was almost impossible, so all the doctors could do was to change the medication every day.  That’s how I changed the medication for three months.  I watched the old man lose weight from 150 pounds to 80 pounds, and there was nothing the whole hospital could do. One day I came to the bedside of the old man again to find the bed empty, the first is a surprise, can not be the old man was discharged from the hospital? Second reaction: not good. Otherwise, the nurse came and told me that the old man had left in the early hours of last night. An old man who used to push a cart to the front line to send clothes and quilts to the Eighth Route Army spent her life in a cold hospital bed, just because of a carelessness at the beginning.  If the children of the elderly can be a little more active, if the operation can be done a little earlier, if …… more if can not return the life of the elderly. It was also the first time I really felt the horror of a hip fracture.  Remember the numbers: hip fractures have a one-year mortality rate of nearly 1/3, a lifetime disability rate of nearly 1/3, and 1/3 of the elderly will never be able to stand and walk. This is more dangerous than most malignant tumors! It’s terrible!  Three 1/3 is enough for us to wake up and pay attention, if an elderly person at home accidentally falls and has a hip fracture, don’t hesitate anymore, doctors will fight with you against the disease!
