A 47-year-old woman in Fuzhou suffered from an extremely rare congenital right-sided gastric protrusion into the right thoracic cavity, causing her to cough up blood every day for more than 20 years. Recently, the cardiothoracic surgery department of Fuzhou General Hospital of Nanjing Military Region successfully performed a difficult right lower lung and thoracic cavity remnant gastrectomy for her at great surgical risk, and the coughing blood symptoms were eliminated after the surgery. It is understood that 24 years ago, the patient Ms. Liu discovered during a physical examination that her stomach did not grow in the left side of the abdominal cavity like ordinary people, but incredibly grew in the right side of the thoracic cavity and was closely adhered to her right lower lung. At that time, after the diagnosis of congenital right-sided stomach protruding into the right side of the chest cavity, she underwent a partial gastrectomy and gastric repositioning in a hospital, but after the operation, pneumonia occurred and she coughed up more blood and needed to rely on drugs to compulsorily stop the bleeding. In mid-April this year, Ms. Liu came to the cardiothoracic surgery department of Fuzhou General Hospital for treatment. After consultation and discussion, experts in the department concluded that the patient’s stomach organs and right lower lung were adhered together, causing gastric juice to keep corroding the right lower lung, and thus the patient would cough up blood and secretions from the stomach every time she coughed, which led to more than hemoptysis. Without surgery, the gastric juice would continue to corrode the lungs until the entire right and left lungs were completely corroded. The only way to treat the symptoms was to perform a right lower lung and right thoracic remnant gastrectomy as soon as possible. During the operation, Yang Shengsheng, the director of the Cardiothoracic Surgery Department of the General Hospital, personally took the lead, and with the assistance of Dr. Zhang Rongliang and Dr. Liu Hongtao, carefully peeled off the right lower lung and the right thoracic gastric remnant, which had become one in the patient’s body, and completely excised the lesion, finally taking more than 5 hours to complete the whole operation. As the patient was bleeding a lot during the operation, the specialist gave 800 ml of suspended red blood cells, 330 ml of plasma and 10 units of platelets during the operation, and continued the comprehensive treatment of phlegm, asthma, hemostasis, anti-inflammation and nutritional support after the operation, so that the patient recovered smoothly and the hemoptysis symptoms were completely eliminated.
