Celiac disease is not a separate disease, but a manifestation of chronic cervicitis, which is caused by human papillomavirus infection. As a disease condition, cervicitis includes the following three specific lesions: cervical erosion, cervical cysts and cervical polyps, the most common of which is cervical erosion. Early treatment of cervicitis and cervical erosion is crucial. When cervical erosion occurs and contains a large number of white blood cells, the incidence of cervical malignancy in women with cervical erosion is usually 5-10 times higher than in normal people. The main cause of cervical malignancy is due to repeated damage to the cervix and long-term inflammatory cell stimulation. In case of moderate to severe cervical erosion, especially granular and papillary cervical erosion, the local immunity of the cervix is reduced. There are many treatments for cervical erosion, medication, physical therapy and surgery, which can be chosen appropriately according to the degree of erosion.
The cervical erosion can be divided into 3 types: type 1: at the beginning of inflammation, the erosion surface is only covered by a single layer of columnar epithelium with a flat surface, called simple erosion; type 2: subsequently, due to excessive epithelial hyperplasia, accompanied by interstitial hyperplasia, the erosion surface is uneven and granular, called granular erosion; type 3: when the interstitial hyperplasia is more significant, the surface is more serious uneven phenomenon and papillary, called papillary erosion The type III: when the interstitial hyperplasia is more significant, the surface is more severe and papillary, called papillary celiac disease.
According to the size of the erosion area can be divided into 3 degrees: mild refers to the erosion surface less than 1/3 of the entire cervical area; moderate refers to the erosion surface accounted for 1/3-2/3 of the entire cervical area; severe refers to the erosion surface accounted for more than 2/3 of the entire cervical area.
Choose the right treatment method for cervical erosion
Generally speaking, if it is a small area of mild celiac disease, you can choose some local medication; but if it reaches moderate degree or above, you must start treatment, and there are two methods of treatment: medication and physical therapy. It is important to note that for patients with moderate and severe cervical erosion, medication is not effective and takes a long time and costs a lot, so it is recommended to use physical methods to obtain better results. Here we mainly introduce some physical methods.
1.Laser treatment
Advantage: Most of the carbon dioxide laser is used to irradiate the erosion surface with a beam of light, causing the erosion tissue to char and crust, and new squamous epithelium can grow after the crust falls off. Laser treatment has anti-inflammatory, pain-relieving and tissue-repair promoting effects. It is easy to grasp the depth of cauterization during the treatment because the laser knife does not touch the cervical tissue. It is usually curable in 1 time.
Discomfort: There may be a burning sensation in the vagina during the treatment. Vaginal discharge or bloody discharge may occur 1 to 2 weeks after treatment. The healing time is usually about 1 month. There will be no special discomfort during the operation. The duration of the operation is about 5 minutes, and a few people have bleeding when they are de-obsessed.
2.Cryotherapy
Advantage: The special rapid freezing device is used to freeze, necrotize and shed the diseased tissue of the cervical erosion surface. The commonly used freezing agent is liquid nitrogen, so that the temperature of the freezer head can be lowered to minus 196 degrees Celsius and placed on the surface of the cervix, so that the erosion tissue freezes and necroses, falls off, and then grows new epithelial tissue. Cryotherapy does not form scars, so cervical stenosis does not usually occur and is more suitable for women with fertility requirements. In addition, freezing can make the local blood vessels contract and stop bleeding, so the patient rarely bleeds after treatment. 1 time can cure.
Discomfort: Freezing can reduce the sensitivity of the nerves and thus has anesthetic and analgesic effects, so the treatment generally has no adverse reactions, but a few people may experience dizziness, lower abdominal distension and other reactions due to low-temperature stimulation, which may cause plant nerve dysfunction. The vaginal discharge is high after treatment and lasts for 2-3 weeks.
3.Microwave therapy
Advantage: The use of microwave to produce thermal effect, prompting the cervical tissue around the protein coagulation, play a cauterization, coagulation hemostasis, anti-inflammatory, sterilization and wound healing effect. The doctor’s field of vision is clear and easy to operate when using microwave therapy.
Discomfort: There is usually no pain during the operation, little bleeding, uniform depth of the treatment surface, and fast wound cell repair after the operation.
4.Bohm light treatment
Advantage: Through the photothermal compound effect to make the erosion tissue protein coagulation, cell denaturation necrosis, crust off, but also promote the regeneration and repair of the body, so as to achieve the purpose of treatment.
Discomfort: No damage to the deep blood vessels during the operation, no larger amount of bleeding after the operation, simple operation, less pain, and the treatment is not affected by the intrauterine device.
5.Electric ironing
Advantage: A special electric iron is used to cauterize the tissue on the erosion surface to make it necrotic and fall off, but a certain depth needs to be reached during the treatment so as to achieve an effective treatment effect. During the treatment, the iron is touched on the erosion surface and slightly pressurized, and moved back and forth from the inside out to the left and right until it slightly exceeds the erosion surface.
Discomfort: The procedure may be mildly painful, but tolerable. There may be more vaginal discharge for 2-3 days after the procedure, and there may be a small amount of vaginal bleeding for 2 weeks. After 2-3 weeks, the wound surface will be decrusted and the squamous epithelium will start to repair.
6.Ablation knife (BBT)
Advantage: Under the dynamic observation and guidance of B ultrasound, the RF treatment source is passed through the vagina, cervix and other natural cavities by the self-coagulation knife, which can accurately locate the lesion site and automatically and precisely control its treatment power, time and treatment range, so that the lesion local tissues can produce biological high heat effect, then the erosion surface can be coagulated, degenerated and necrosed, or the lesion tissue of the endometrium can be ablated and finally be absorbed by normal tissues. It will be absorbed by normal tissue or discharged automatically.
Discomfort: There will be slight pain and discomfort, but little bleeding during the operation.
7.LEEP knife
Advantage: It is actually a cervical circumcision, so the patient is the one with heavy cervical erosion, and the high-frequency electric wave generated by the electrode tip, after contacting the body, due to the impedance of the tissue itself, absorbs the high-frequency electric wave and generates high heat instantly to finish the treatment of cervical erosion. The advantage is that a very fine surgical result can be achieved. It does not produce tissue pulling or charring and causes little damage to the surrounding tissues.
Discomfort: Since anesthesia is administered during the procedure, there is no pain, but there may be a slight discomfort and the procedure takes only 3 to 5 minutes on average. There is little pain and little chance of scarring.
Precautions
The treatment should be performed 3-7 days after menstruation because the cervix is invasive after the treatment and is prone to infection if it coincides with menstruation. Therefore, patients should come to the hospital as soon as possible 3 days after menstruation.
Intercourse is forbidden 3 days before the operation, and those with acute inflammation of the reproductive tract should be cured before proceeding.
After the treatment, the discharge increases or even a large amount of fluid is discharged. Pay attention to the cleanliness and hygiene of the vulva and wash the vulva regularly, but do not use vaginal suppositories or perform vaginal douching by yourself. Change underwear regularly and use sanitary pads if necessary.
There may be a small amount of bleeding when the scab falls off 1-2 weeks after treatment. If there is a lot of bleeding, you should seek medical attention promptly.
Avoid sexual intercourse as much as possible until the wound is completely healed. The mechanical stimulation from sexual intercourse can affect the healing of the cervical wound and may also cause vaginal inflammation, thus affecting the repair of the normal tissue of the cervix.
One or two months after the treatment, you should go to the hospital for a review of the healing of the cervical wound and decide on further treatment under the guidance of the doctor
Pre-treatment preparation
Regardless of the method chosen, the possibility of cervical cancer should be ruled out first before treatment begins, because early cervical cancer is very similar in appearance to cervical erosion and cannot usually be identified with the naked eye, and a cervical smear is needed to understand the possible existence of cervical malignancy. Because if it is cervical cancer and physiotherapy is mistakenly used, it not only delays the treatment of cervical cancer, but also may make the cancer cells spread. Before treatment, it is also necessary to exclude the presence of inflammation of the vagina and cervix, especially combined with sexually transmitted diseases, to avoid aggravating the inflammation or affecting the healing of the cervical wound.
It is especially important to remind that ultrathin cervical smear-TCT has now replaced the traditional cervical smear and is of particular importance in the early detection and treatment of cervical cancer. TCT should be routinely performed if financial conditions permit.
