When symptoms such as dizziness occur as soon as you squat down and get up, this is the time to consider the possibility that the patient has postural hypotension. To clarify whether there is postural hypotension, we measure the blood pressure when it squats, and then measure it again when it stands up. If the blood pressure has decreased very significantly, by at least 20 mmHg systolic or 10 mmHg diastolic, and there are already symptoms of dizziness and cerebral hypoperfusion, including blurred vision, fatigue, weakness, cognitive impairment, and pain in the back of the neck, consider the occurrence of postural hypotension. Postural hypotension is relatively common in the elderly, mainly due to the use of drugs such as tricyclic antidepressants, diuretics, nitrates and alpha blockers for prostate enlargement.
