Every parent, when their baby child is born and found to have hand and foot deformities, is very distressed because they feel that their child is no longer perfect and are even willing to transplant their own hands and feet over, which is actually a normal psychological reaction that doctors, who are also parents, empathize with. After all, parents are adults, and whether their children are physically perfect or not, they (they) are little angels and no one should blame them anymore. First of all, parents must first pass this psychological barrier, objective and calm look at this matter, only then, it is possible in the future on the road of life, to channel their children’s psychological, hand in hand to tide over the difficulties, so that the little angels of the psychological health growth. After years of being an orthopedic hand surgeon, I deeply realize that psychological health is the most important, and he is stronger than a hundred times the physical defects. In the clinic, almost all parents are concerned about the following questions, I list them one by one, explain, but hopefully will help. First, why is the child like this? This is a very complex and difficult question to answer precisely, even today with such advanced technology. Generally speaking, during the 2-3 months of the mother’s pregnancy is the period when limbs such as hands and feet are formed. If the fetus’ development is disturbed at this time, it is likely to form malformations of hands and feet, and may be accompanied by abnormal development of internal organs such as the heart. What interferes with normal development? It could be hereditary, something wrong with the genetic code, or it could be a viral infection, taking sensitive drugs or other teratogenic drugs or other factors that are hard to say. This is why early pregnancy is so important for eugenics. Second, will the parents still look like this when they have another child? My understanding is that if it is not hereditary, i.e. not inherited from the parents’ genetic abnormality, but caused by external factors, i.e. environmental factors as mentioned earlier, it usually will not. Third, when is the right time to operate? This should be analyzed on a case-by-case basis, and there is no one-size-fits-all approach, but there is a general rule. For example, if the thumb is deformed, the international mainstream view is to put it around one year old, because this is the stage when children use their hands to explore and understand the world, and the functions of the hands are developing rapidly, so surgery too late may affect the establishment of hand functions, while too early increases the difficulty. If the syndactyly does not affect the development of the fingers, surgical treatment can be considered after one and a half years of age, but if it affects the development of the fingers, treatment is needed before one year of age. As for ring band syndrome, if it affects the development of the limb, then the contracture factor should be removed as early as possible. However, in any case, in our experience and according to international opinion, all surgical treatment should be completed before school age. Adolescence is the most sensitive period for children, and there are many difficulties in opening surgery at that time, and problems such as the child’s resistance and psychological sensitivity need to be faced and solved. Fourth, how should the treatment be done? Compared with the rapid development of technology, the development of medical technology itself, especially surgical technology, is not so fast. When I returned from my study in the United States, I realized that in this world, there will always be so many difficult problems that are beyond human power, although we are working hard for this every day. For some relatively easy to solve the problem, there are many methods, each has its own advantages and disadvantages, when we look at the strengths of a method, inevitably have to face its weaknesses, there is a balance here, excessive pursuit often want to speed but not achieve. Therefore, when over-rendering how beautiful a method, itself is not rational and objective enough, because there is no perfect thing in the world.
