How to determine the severity of neck and shoulder pain and low back pain and the choice of treatment

It’s been said that two things are usually inevitable in a person’s life: death and taxes, when in fact they should also include neck and shoulder pain and low back pain. As many as 80 percent of adults will eventually feel this pain, and it is the leading cause of outpatient visits for hospitalization or incapacity. Thankfully, most patients with neck and shoulder pain and low back pain will recover significantly and quickly. The spine, as the central hub that connects and supports the entire body, moves and bears weight for years, just like machine parts that have worn out over the years. As we age, problems almost inevitably arise in the most active cervical spine and the most weight-bearing lumbar spine – aging, disc degeneration, vertebral instability, osteophytes, spinal stenosis; in addition, congenital deformities, poor work posture, overexertion, occasional or repeated trauma, these conditions will accelerate and exacerbate this change and can lead to early onset of lumbar disc herniation. This explains why cervical and lumbar spondylosis are more common in desk workers, drivers, heavy laborers, and athletes. Changes in the spine itself are not the cause of all pain and discomfort; soft tissue factors such as muscle strain and myofasciitis are also common. Inflammation and tumors, in addition, are also terrible causes of spinal pain, but the good news is that this is much less common compared to trauma and degeneration. Neck and shoulder pain and low back pain accompanied by wasting, fever, night sweats, or with severe pain and progressive worsening of symptoms at night, should alert to spinal tuberculosis and tumors. Patients with cervical spondylosis and lumbar spondylosis experience a variety of painful sensations, but one should grasp a boundary and distinguish between conditions that are not naturally reversible and those that are capable of rapid recovery. Generally speaking, typical limb weakness and numbness or shoulder and arm radiating pain as well as sciatica indicate that a herniated, degenerative disc in the cervical or lumbar spine may have compressed the spinal cord or nerve roots. The compression is real and sometimes requires surgical removal to solve the problem. Other than that most neck, shoulder and back pain can be relieved by conservative treatment or natural recovery. Conservative treatment includes rest, traction, physical therapy, massage, closure, medication and other therapies, the desire of which is only to prevent aggravation of the condition and speed up the natural recovery process. A small number of serious, non-surgical treatment is ineffective, and the exact cause can or must be selected for surgical treatment. Of course, it is still extremely necessary to see a specialist when you have a spinal condition, and these doctors will have to study and prepare for at least 10 years in order to solve your confusion. Again, for cervical spondylosis, surgery is almost mandatory for the spinal type, and 90% of non-surgical treatments are effective for the nerve root type. Of the common lumbar spine conditions, unlike herniated discs, the more effective treatment for lumbar spinal stenosis is surgery, not conservative treatment. Often, conservative treatment is only used as a secondary or waiting treatment. In the prevention of cervical spondylosis and lumbar spondylosis, it is important to remove the causative factors and avoid poor working positions, overstrain, and trauma as mentioned earlier, and physical therapy and interval exercises can be much more useful than generally thought. The flexibility and strength of the posterior cervical, abdominal, and back muscles are extremely important factors in strengthening and ensuring the stability and support of the spine, just like a weak flagpole that needs to be reinforced and fixed by steel cables on both sides. Appropriate exercise is beneficial, to the extent that it does not aggravate symptoms and can be tolerated. Excessive activity is counterproductive and is a strain of your own making. It is important not to have a one-sided understanding and pursuit of “life is in motion”.
