Does difficulty in urination necessarily mean prostate enlargement?

  Prostatic hyperplasia is very common clinically and often causes symptoms such as frequent urination and difficulty in urination, but the presence of difficulty in urination also requires consideration of the presence of the following diseases: 1. Neurogenic bladder Can cause difficulty in urination, urinary retention or urinary tract infection, and other symptoms similar to prostatic hyperplasia. However, patients with neurogenic bladder often have a history and signs of significant neurological damage, such as sensory and motor impairment of the lower extremities, constipation, fecal incontinence, decreased or lost sensation in the perineum, and relaxation and decreased or absent contraction of the anal sphincter. The prostate is not enlarged on rectal examination.  2, bladder neck sclerosis also has symptoms of bladder neck obstruction, but the onset is mostly seen in adolescents; the prostate gland is not enlarged on rectal examination; on cystoscopy, the posterior lip of the bladder neck is obviously sclerotic, forming a “median ridge”, protruding into the bladder, and the urethral orifice is deformed.  3.Urethral stricture Symptoms such as difficulty in urination, fine urine flow or urinary retention, but there is a history of urethral injury and urethral infection; the prostate is not enlarged on rectal examination and is obviously displaced upward; on urethral probe examination, the probe is obstructed at the stricture, and cystourethrography can show the stricture.  4.Prostate cancer The prostate gland is enlarged and may show symptoms similar to hyperplasia. However, there are hard, inelastic nodules in the prostate on rectal examination; serum acid phosphatase, alkaline phosphatase and PSA can be increased, and cancer cells can be found on prostate biopsy; the prostate gland is enlarged on B-mode ultrasonography, with discontinuous envelope reflection and unclear boundaries.  5, prostate stones rectal diagnosis prostate enlargement, there are frequent urination, urinary difficulties and other symptoms. The nodules can be found in the fingertip examination, and there is a feeling of stone rubbing; the pelvic X-ray can see the shadow of stones in the prostate area.  6.Bladder cancer near the bladder neck is clinically manifested as bladder outlet obstruction, often with symptoms such as difficulty in urination. However, the patient has painless hematuria and urine exfoliative cell examination can detect cancer cells. Cystoscopy can directly see the location, size, number and infiltration degree of tumor, and if biopsy is taken at the same time, the nature of tumor can be clarified.
