Round red spots that do not hurt or itch may be caused by pityriasis rosea, congenital hemangioma, and stage II syphilis rash.

3. Stage II syphilis rash: it is a disease caused by the infection of syphilis spirochete, which is mainly transmitted by sexual contact. Due to the lack of standardized treatment, syphilis rash will appear after entering the second stage, which can be manifested as a red patchy rash or maculopapular rash appearing all over the body, and even on the palms of the hands and soles of the feet, which can be painless and itchy.
