Inflammation of wisdom teeth

Wisdom tooth inflammation, also known as pericoronitis, is an inflammation of the tissue surrounding the wisdom tooth (third molar) and is common in the lower jaw. Acute inflammation is characterized by severe pain, swollen gums, throbbing teeth, bad breath, loss of appetite, and headache, while chronic inflammation is often less painful and patients may have no significant discomfort. The main cause of inflammation of wisdom teeth is due to the inability to erupt properly, which manifests as partial eruption, inclined growth or obstruction caused by complete encapsulation in the bone. In addition, bacterial infection, poor oral hygiene, pregnancy, emotional stress, and other factors can directly or indirectly cause wisdom tooth inflammation. The conventional treatment plan is anti-inflammatory and pain relief, and wisdom tooth extraction treatment can be performed after the symptoms of acute inflammation are relieved. The first step is to remove plaque, calculus, food debris and necrotic tissue by rinsing with hydrogen peroxide, saline and chlorhexidine solutions to relieve local inflammation. If the patient forms an abscess around the wisdom tooth, incision and drainage should be performed as soon as possible to prevent the spread of inflammation. Then, further anti-inflammatory medications such as iodine glycerin and minocycline hydrochloride ointment can be applied locally. For patients with persistent inflammation or severe pain, systemic application of amoxicillin, metronidazole, tinidazole, clindamycin, cefixime, ibuprofen, and hypericum hydrobromide tablets can be used to relieve pain as prescribed by the doctor. After the patient’s symptoms have subsided, treatment with incision and drainage, pericoronal gingival flap removal or wisdom tooth extraction can be performed as appropriate. Along with active treatment, patients should also improve their living and eating habits. They should make sure to brush their teeth meticulously in the morning and before going to bed to ensure that every surface and crevice of the teeth is cleaned, and they can rinse their mouth after meals to reduce bacterial growth that can cause infection.
